• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-81-013 <br />INSPECTION DATE 7/8/99 INSPECTOR'S INITIALS RCO <br />groundwater and well levels. <br />Final discussions during the inspection involved mining phases, concurrent reclamation, determining adequate bond <br />amounts, and the citizen objections received by the Division. The operator is encouraged to communicate with all <br />objectors, in an attempt to resolve these matters relative to their topics of objection. This office will continue to deal <br />strictly with topics within the jurisdictional limits. <br />One final item was discussed, involving the name of the permittee vs. the name of the applicant in this conversion. The <br />Division still considers the permittee to be "John, Terry and Ward Nichols", while "Nichols Gravel Pit" has appeared on all <br />correspondence in this conversion to-date. At present, this office must consider these two entities to be different parties, <br />If John Nichols wishes to continue to submit aoolication materials toward this conversion, this office must receive some <br />written statement from the "oermittee" that John Nichols of Nichols Gravel Pit may submit any necessary materials on the <br />behalf of the oermittee. If such statement is not received, this office must return to contacting Terry Nichols for all <br />matters. After the conversion is finished, as the operator may remember from separate correspondence from the Division <br />in May 1999, the permit may be transferred to "Nichols Gravel Pit." <br />There were no further items observed or discussed, and no problems noted during this inspection.
