Cottony <br />Division Director <br />Natural Resource Trusiee <br />It has come to the attention of the Division of Minerals and Geology that a problem noted in the past <br />DMG inspection report either has not been corrected, or has been corrected, but documentation of the <br />corrective actions taken were not submitted to our office. <br />Please see the enclosed inspection report for the Jesse Lee Gravel Pit (Permit No. M-1984-043) issued as <br />a result of the September 29, 2004 inspection of the site. Problems noted in this report, that our records <br />indicate remain unresolved, have been highlighted. In particular, the operator has failed to post a mine <br />identification sign and submit proof to the Division in the Form of photographs. <br />To avoid potential future enforcement actions for non-compliance with State Statutes, Rules and <br />Regulations, and/or conditions specified in your permit for this site, documentation of compliance with <br />the corrective actions specified for each of the indicated problems must be submitted to our office by May <br />2, 2005. <br />If you need additional information, please contact Erica Crosby at the Division of Minerals and Geology, <br />1313 Sherman Street, Room 215, Denver, Colorado 80203, telephone number 303-866-4943. <br />Sinsare~ly, <br />Erica S.
