(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2007-014 <br />INSPECTION DATE 06/0412007 <br />INSPECTOR'S INITIALS DEZ <br />This pre-operational inspection of Permit Number M-2007-014 (Byzantine Quarry) was conducted by Deb E. Zack of the <br />DRMS. High Country Pipeline representatives Bill Williams and Roland Smith, and Jack Clark of Clark Mining Services, LLC <br />were present during this inspection. The site is located approximately 3 miles northwest of Canon City, CO. This operation will <br />be an expansion of a current 110(c) operation. Therefore, there are currently approximately 9.8 acres of mining related <br />disturbance (see photo 1). <br />Prior to inspecting the site, an impromptu meeting was held by all of the parties listed above. At this meeting the adequacy <br />review letter and the requests for additional information were discussed. During the meeting, this inspector was provided with <br />revised maps of the proposed mine site. <br />Once onsite, this inspector observed that the mine entrance sign was posted, as were the current and proposed permit <br />boundary markers (see photos 2-4). All activities and completed work at the time of inspection appeared to follow the guidelines <br />and regulations set forth by the state and in the mining and reclamation plans for the pit. At the time of this inspection the site <br />appeared stable. There appeared to be no offsite erosion or activities, and no long-term (greater than 72 hours) water retention <br />was observed. <br />Responses to this inspection report should be directed to Deb E.
