<br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M 14HR-117 <br />INSPECTION DATE: aiigiict 1~, znm <br /> <br />INSPECTOR'S INITIALS: "/~~ <br />1 Sian: Located at the entrance to the property Looks Good. <br />2 Boundaries: Not inspected on this inspection tour. <br />3 Tailings Impoundment: The tailings dam and catch basin appear to be well vegetated and stable. Water is <br />being retained in a pond on the tailings. A Center Divot sprinkler is beino installed to dispose of this water by <br />spraying it on the vegetation adjacent to the pond. This will dispose of the water by evaporation to the air and <br />absorption by the vegetation, Once the this water is removed and water stops percolating from the tailinos. <br />(approximately two years) the dam will be breached by constructing a spillway which wil drain surface water <br />across the tailings and down the spillway. As the tailings continue to dry, their stability and the stability of the <br />dam will increase. <br />4 Waste Dump: Revegetation of the waste rock dump is progressing well with several scecies of grass <br />established and some sane moving in. <br />5 West Pit: Several hundred trees have been planted in the west pit area. They appear to have an excellent <br />survival rate, and there is some consideration being given to thinning them by transplanting some trees to other <br />locations on the site. <br />6 West Pit: The original seep area from the east end of the west pit is drv indicating that the operator is <br />maintaining the water level at an appropriate elevation. <br />7 Diversions and Sedimentation Structures: The mine site experienced a large precipitation event in the oast <br />couple weeks which was reportedly conducted along the diversion ditches to the sediment ponds demonstrating <br />the effectiveness of the system. <br />8 Water Treatment: The operator is continuing to treat the water by reverse osmosis. but is evaluating a passive <br />system using activated alumina to remove fluoride. This should be the subiect of a technical revision. <br />9 Water Treatment: The operator indicated that they have a pump stuck in a well. Ther have attempted to <br />remove it. push it down and are unable to retrieve it. They plan to drill another well to replace the one <br />incapacitated by the stuck pump The operator must provide the location and completion information to <br />the Division of Minerals and Geoloav after the well is completed. <br />10 Roads: The operator is planning to reclaim the haul road to the millsite by Dulling dowr the berm on each side <br />to narrow the road to a more representative width compared to the other county roads. <br />it Overall Site: The overall appearance of the site indicates that the operator is committed to timely reclamation <br />of the site and is doing an excellent job.
