(Page 2)
MINE ID # OR PROSPECTING ID #: na-~nm-mn

INSPECTION DATE: s-~~-m INSPECTORS INITIALS: ~_
OBSERVATIONS
The Division conducted apre-operational inspection of the site on 5-23-01. The site is proposed to bean 80-acre phased
construction materials, 112 operation located approximately 7 miles North of Matheson Colorado. Present during the
inspection was Mr. Tim Sheridan, of Elbert County Road and Bridge. The entire'site was driven and inspected, permit
boundary markers were properly displayed. During the inspection, a brief discussion was held regarding the three dry
gulches located with in the permit area, orienting South to North and the fence located along the East Side of the permit
area. Mr Scheridan stated that a fifty feet buffer would be maintained in lieu of an agreement with the adjacent
landowner. In fact he sated that he had talked to the landowner and that he has no objection to the county operating
with in fifty feet of his fence. With that in mind and considering the size of the operation, the fifty-foot set back should be
more than adequate to protect the fence from adverse impacts. The fifty-foot buffer for the dry gulchs should also be
more than adequate to protect impacts to the gulches due to mining. Permit application sign was properly posted near
the access to the property. The site was as described in the permit application.
I & E Contact Address
NAME: Mr. Tim Sheridan
OPERATOR: ELBERT COUNTY ROAD AND BRIDGE
STREET: ROQ UTE AVENUE
CITY/STATE/ZIP: SiMLA, ('O R0R35
JAMES STEVENS
cc: ^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ SE
^ WQCD (CH)
^ OTHER
