<br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M 2000-087 <br />INSPECTION DATE: 10-02-01 INSPECTOR'S INITIALS: rR5 <br />OBSERVATIONS <br />This was an inspection of the Milliken Resource site conducted by Gregg Squire of the Division of Minerals and Geology. <br />This inspection was done as aPre-operational inspection in response to an application for a Permit Amendment. The <br />permit amendment was filed for this site on 08/23/01. The original permit was approved on September 8, 2000 for 205.2 <br />acres. The current amendment was applied for on August 23, 2001. The amendment proposes some changes in the mine <br />areas, a change in the location of the processing plant and the addition of another 287.6 acres to the permitted area. <br />As part of the inspection the original permitted area was toured. No mining or other disturbance by the operator was noted. <br />Correspondence from the Colorado DOW indicated that a Preble's Meadow Jumping Mouse had been captured on the site, <br />and that there was a winter roosting site for Bald Eagles on the site. These locations were inspected. The general mine plan <br />was reviewed and the old and amended permit boundaries were located. The public notice sign was found but had been <br />knocked down, it should be reported. It was also observed that no Permit ID sign had been posted on the area were the <br />permit had been issued, a sign should be posted, and a photo of the sign sent to the Division to verify that it has been put up. <br />While reviewing the maps of the amendment application it was also noted that there were no signatures on the maps and that <br />the permit boundaries were not labeled.
