• (Page 21 • <br />MINE Ip k OR PROSPECTING ID rY M-1978-116 <br />INSPECTION DATE 1!17/01 INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred as part of DMG routine monitoring of permitted sites. The SM-18 Mine <br />is located approximately 2 miles north of Uravan, within DOE lease block AT105-1)-60.8-C-SM-18. <br />Permit History The original 112 permit was issued on October 31, 1979, to Cotter Corporation, <br />for 14.9 acres affected area. Operations went into temporary cessation on October 31, 1980. On <br />August 22, 1985, the 5-year extension of temporary cessation was approved. On August 31, <br />1990, TR-01 was approved automatically, addressing intermittently active status for the permit. <br />On February 28, 2000, DMG approved TR-02, addressing the requirements of Rule 4.2.1(8) for <br />operations having a reclamation bond with a federal land management agency. Under TR-02, DMG <br />accepted the $37,000 DOE bond as financial warranty. Pursuant to Rule 4.2.1(81, DMG holds <br />$100 financial warranty. <br />Portal Area Affected area at the portal was approximately 2.15 acres. The mine identification <br />sign was posted. Access into underground workings by unauthorized individuals is controlled by <br />locked steel gate at the portal entrance. Vegetative cover for the topsoil stockpiles could not be <br />verified due to snow cover. However, during the last inspection, the DMG inspector observed that <br />the vegetative cover appeared sufficient to protect the stockpiled topsoil. <br />Venthole Areas The ventholels) were not inspected. The permit is approved for 6 ventholes. <br />However, DMG documents indicate that only 1 venthole, 7' diameter, has been constructed. <br />Please clarify the number of ventholes constructed and their locations. <br />Financial Warranty On February 10, 2000, DMG calculated the cost of reclamation totaling <br />$17,180. Under TR-02, DMG has accepted the $37,000 bond, held by DOE, as financial warranty. <br />Therefore, pursuant to Rule 4.2.1(81, and because the amount of bond held by DOE exceeds the <br />calculated cost of reclamation, DMG holds $100 financial warranty.
