1 ~ <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-i aRn-~aa <br />INSPECTION DATE: a-~a-n~ INSPECTOR'S INITIALS: aK <br />The Division conducted a monitoring and construction progress inspection of the site on 9-14-01. Present <br />during the inspection were Mark Tidquist and Phil Barnes of CC&V. Highway relocation work continuing, a <br />portion of the highway was being paved at the time of the inspection. Inspected the small area south west <br />of the crusher that was lined as part of the Phase IV LEACH PAD AREA, AS APPROVED IN AMENDMENT # 8, <br />NEAR THE FUTURE Load out Bin area. According to Mark Tidquist, CC&V WANTED TO PLACE LINER IN THE <br />AREA, SO IT CAN BE USED AS STAGING FOR THE PHASE IV PAD AREA CONSTRUCTION. The lined area <br />appeared to be properly anchored in accordance with liner placement specification as approved in <br />Amendment # 8. <br />Inspected the Phase III leach pad area. Diversion work was continuing. The final head wall for the <br />diversion CMP was in place. No major problem noted. <br />Inspected the Phase II area pregnant solution collection system and recorded the following readings from <br />the high and low volume solution collection transducers. <br />Low volume transducer #1 was recorded at .12 and # 2 was at .22 <br />High volume transducer # 4 was recorded at 16.2. # 5 was at 22.3 and # 6 was at 25.6 (all measurements <br />are in hydrostatic feet of head) <br />Inspected the underdrain area, the west side diversion Arequa Gulch diversion ditch has been cut to allow <br />the probable placement of the storm water return recovery pipe in the center while also acting to act as a <br />riprapped natural channel. All three UNDERDRAIN PIPES were discharging, no samples collected. The water <br />from the North underdrain pipe was being pumped in to the external pond to be utilized as make up water <br />in the ARD building. Mr Barnes stated that CC&V had the rights to the water, and that all the water used in <br />the facility is properly recorded. He was informed again, that the Division had sent the previous inspections <br />reports to the State Engineer and CDOH &E, which indicated the diversion of WATER from the underdrain <br />discharge into the external pond. <br />Highway relocation bridgework was continuing, no major problem noted in the area. <br />Inspected the external pond, it was holding a lot of water. As stated above, water was being pumped from <br />the North underdrain into the pond.
