~I <br />(Page 4) <br />u <br />MINE ID # OR PROSPECTING ID # M-97-094 <br />INSPECTION DATE 02/03/98 INSPECTOR'S INITIALS AJW <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The operator has failed to address the requirements of 04/30/98 <br />Rule 6.5(4), specifically, at sites where blasting is PB or PV: PB <br />part of the proposed mining or reclamation plan, the TOPIC(S): EX <br />applicant shall demonstrate through appropriate <br />blasting vibration, geotechnical, and structural <br />engineering analyses, that off-site areas will not be <br />adversely affected by blasting. <br />Corrective actions are to: <br />1) Immediately cease all blasting and <br />submit a technical revision to the <br />Division which clearly demonstrates <br />compliance with Rule 6.5(4). <br />The operator failed to fully address how the site will <br />remain stable during mining and reclamation operations, <br />specifically, no plan was submitted to address wind <br />erosion and fugitive dust concerns. <br />Corrective actions are to <br />1) Submit a plan in conjunction with the <br />above technical revision which <br />addresses proposed measures to be <br />implemented which will address any <br />potential wind erosion and fugitive <br />dust concerns as required by C.R.S. <br />34-32.5-116(4)(j). <br />04/30/96 <br />PB or PV: PB <br />TOPIC(S):SC, <br />MP, OD <br />The operator failed to clearly describe the procedures 04/30/98 <br />that will be used to load out the finished product onto PB or PV: PB <br />trucks for shipping offsite, specifically, Siloam road TOPIC(S): MP, <br />was not mentioned as a road that would be used for the RD, <br />mining operation. Rule 6.3.3(g) specifically requires <br />descriptions of roads which will be used in the mining <br />process. <br />Corrective actions are to <br />1) Modify the approved mining plan in <br />conjunction with the above technical <br />revision by providing a description <br />of all roads which will be used in <br />Che mining and reclamation plans. <br />
