The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE The operator has failed to address the requirements of 04/30/98 Rule 6.5(4), specifically, at sites where blasting is PB or PV: PB part of the proposed mining or reclamation plan, the TOPIC(S): EX applicant shall demonstrate through appropriate blasting vibration, geotechnical, and structural engineering analyses, that off-site areas will not be adversely affected by blasting. Corrective actions are to: 1) Immediately cease all blasting and submit a technical revision to the Division which clearly demonstrates compliance with Rule 6.5(4). The operator failed to fully address how the site will remain stable during mining and reclamation operations, specifically, no plan was submitted to address wind erosion and fugitive dust concerns. Corrective actions are to 1) Submit a plan in conjunction with the above technical revision which addresses proposed measures to be implemented which will address any potential wind erosion and fugitive dust concerns as required by C.R.S. 34-32.5-116(4)(j). 04/30/96 PB or PV: PB TOPIC(S):SC, MP, OD The operator failed to clearly describe the procedures 04/30/98 that will be used to load out the finished product onto PB or PV: PB trucks for shipping offsite, specifically, Siloam road TOPIC(S): MP, was not mentioned as a road that would be used for the RD, mining operation. Rule 6.3.3(g) specifically requires descriptions of roads which will be used in the mining process. Corrective actions are to 1) Modify the approved mining plan in conjunction with the above technical revision by providing a description of all roads which will be used in Che mining and reclamation plans.
