(Page 2) <br />MINE ID # OR PROSPECTING ID # M-99-063 <br />INSPECTION DATE 05/11100 <br /> <br />INSPECTOR'S INITIALS ESC <br />This was an inspection of the Red Wolf Quarry conducted by Erica Crosby of the Division of <br />Minerals and Geology. Luis Vasquez, the operator and permittee was present during the <br />inspection. Weather conditions on site were clear and ground conditions were dry. <br />The 110 Mining and Reclamation Permit was issued to Luis M. Vasquez on August 23, 1999 to <br />mine flagstone on a permit area of 9.9 acres. The site is currently being used as a storage <br />area for pallets and cutting the stone. The area was specifically inspected because it is <br />located directly east of Permit M-86-115 (Quarry F) operated by Hlue Mountain Stone (" BMS" ) <br />No permit overlapping was noted with the BMS quarry. <br />Permit boundary markers were posted to delineate the permit area. The markers appear to be <br />consistent with the approved mine plan map. The Division was unable to locate a mine <br />identification sign on site. Rule 3.1.12 requires a sign be posted clearly visible from the <br />access road with the name of the operator, a statement that a reclamation permit for the <br />operation has been issued by the Colorado Mined Land Reclamation Board and the permit number. <br />See page 3 for compliance due dates. <br />I ~ E Contact Address <br />NAME Luis M.
