(Page 2) <br />MINE ID # OR PROSPECTING ID #: tie-~ ean_~ee <br />INSPECTION DATE: 5_~9_n~ INSPECTORS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the Phase IV PREGNANT SOLUTION COLLECTION AREA. The <br />main purpose of the inspection was to witness the placement of the caissons. On 5.29-03, CCBV contacted the <br />Division around 10:30 A.M about the possibility that the Caisson base plate might be placed in the affernoon. <br />When the Division arrived on site at 1330, Mr Phil Barnes Environmental affirms manger for the site informed the <br />Division that the upper liner of the pregnant solution collection area was cut to drain the water from the rainfall <br />the night before rather than pumping it out as that were doing. At that time Mr. BARNES WAS NOTIFIED THAT SUCH <br />PRACTICES, EVEN THOUGH ALLOWED IN THE ORE STORAGE AREAS, IN THE PAST, IT WOULD NOT HAVE BEEN <br />Recommended BY THE Division, IN THE PREGNANT SOLUTION COLLECTION AREA, BECAUSE IN THE DOUBLE LINED IS <br />ONLY ALLOWED A TWO FEET OF HYDROSTATIC HEAD MAXIMUM AT NAY GIVEN TIME. When the Division allowed <br />the cutting a removing of water in the ore storage areas in the past, that area lower liner, (THE CLAY) WAS <br />EXPOSED AND THE DIVISION AND THE QA/QC MADE SURE IT, THE CLAY LINER MET THE APPROVED SPECIFICATION <br />PRIOR TO PLACING THE GEOMEMBRANE LINER. In this case, the only way to verify the clay liner was not <br />compromised was fo remove the entire lower liner, and expose the clay, which is highly impractical. <br />Affer the discussion, we went to the site, as reported by Phil, there was a cut in the upper liner to drain the water <br />to the leak collection gravel and lower liner and the area was fully saturated, which, means in excess of two <br />feet of HYDROSTATIC HEAD WAS IN PLACE ON THE DOUBLE LINED AREA. According to Terry Mnadziac, the resident <br />and project engineer for CC&V, the water from the lower liner was pumped out and was being used as dust <br />suppressant on site. Two of the West side caisson base plates were already deployed, there was no tension on <br />the liner, and they appeared as was approved in the revised caisson design drawings. <br />While awaiting the placement of the East side caisson placment, the following readings were recorded from the <br />high volume solution collection transducers and piezomefers. <br />PHASE I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS RECORDED AT 29.0 #2 WAS AT 32.0 AND # 3 <br />WAS AT 31.6. PIEZOMETER WAS AT 35.7 <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 22.3 AND #4 WAS AT 15.3 <br />AND ~ WAS AT 22.1. PIEZOMETER WAS AT 28.7. <br />Since it started to rain it was determined that the placement of the other caissons will be conducted tomorrow. <br />Affer the site inspection, the Division sat down with Mr Ron Largent General MANAGER OF CC&V AND EXPRESSED <br />THE CONCERNS ABOUT THE CUTTING OF THE UPPER LINER. He agreed it was a mistake and that they will make <br />sure such incidents are not repeated.
