(Page 2~ <br />MINE ID # OR PROSPECTING ID #: nn_iean_~ee <br />INSPECTION DATE: ~-79-n~ INSPECTOROS INITIALS: aK <br />OBSERVATIONS <br />this was a monitoring artd construction progress inspection conducted by the Division on 6-i9-03. The area had <br />received a lot of rain and pea size hail the night before which was evident at the time of the Inspection. Present <br />during the site inspection were Mr. Terry Mnadzlac and Mr. Mark Tdqulst of CC&V. Inspection began around the <br />Phase IV PSSA AREA, MET AND BRIEFLY DISCUSSED WITH Klm Morrison and Michael Plase of Golder about the <br />project. Due to the heavy precipitation, the area received the night before; the only construction related <br />activity-taking place was the deployment of drain cover and the first liff ore along the WEST SIDE OF THE PSSA <br />AREA. According to Terry Mnadziac, about 97% of the underground remediation work Is completed in the entire <br />Phase IV leach pad construction area. The riser pipes for the pregnant solution area had been extended, in <br />anticipation of the temporary pump and transducer system deployment. The drain cover deployment area was <br />walked and Inspected, no major problem noted. <br />Inspected the EXTERNAL POND, it was holding some water. While in the are, recorded the following readings <br />from the Low Volume Solution Collection transducers. Transducer # 1 was recorded at 11.3 and # 2 was at 13.9 <br />inches of hydrostatic head. <br />Inspected the Phase 1 AND 11 PSSA AREA and -ecorded the following readings from the Phase I AND II HIGH AND <br />LOW VOLUME SOLUTION COLLECTION TRANSDUCERS. <br />Phase I low volume solution collection transducer # 1 read out meter was hit by lightning and was not <br />functioning, however, the pump was on, # 2 was recorded at .06. High Volume solution collection transducer # 1 <br />was recorded at 19.5 # 2 was at 23.0 and # 3 was recorded at 21.6. Feet of head. Piezometer for the area was <br />recorded at 34.0 feet. <br />Phase II low volume solution collection transducer # 1 was recorded at .06 and # 2 transducer meter was hit by <br />lighting and was not functioning, however bofh pumps were operating. According fo Mark, new mefers have <br />been ordered and should be installed as soon as they arrive. High volume solution collection transducer # 4 was <br />recorded at 22.9 # 5 was pt 17.3 and # 6 was at 22.8 feet of hydrostatic head. Piezometer was recorded at 28.7 <br />feet. <br />Inspected the under drain area, both the south under drain and 4 inch pipes were discharging, the north under <br />drain was dry. All the water from the discharge was being pumped back onto the pad. All lower ponds with the <br />exception of the surface water diversion sump were dry. Surface water diversion structures for the facility <br />appeared to be functioning as deigned, given the amount of rain the area had received the last few days.
