f ' <br />(Page 2) <br />~~ MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 12-16-99 <br />INSPECTOR'S INITIALS BK_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the Phase II and construction progress <br />inspection of the Phase III areas on 12-16-99. Present during the inspection was, Mr. Scott <br />Lewis of CCBV. Inspected the pregnant solution collection area. The following readings were <br />taken from the Phase II high volume solution collection trancducers. Pump # 4 measured 26.0 <br />feet of head, Pump #5 measured 27.8 feet of head and Pump # 6 measured 32.1 feet of head. No <br />solution was noted coming out of the temporary 24 inche solution collection pipe for the <br />phase III area. Low volume solution collection system transducers for the Phase III area had <br />the following readings. Pump #! Read .43 feet of headand pump # 2 had .26 feet of head. Both <br />pampa were on at the time of the inepection.Flow rate was measured at 13 gpm. With the <br />exception of the South West underdrain, all others appeared frozen.Inepected the Phase I <br />pregnnat area. The pipe piezometers were in place. Only one tranceducer for the High Volume <br />Collection system was working at the time of the inspection. <br />After the physical inspection, a meeting with the Mayor of Victor Mr. Jim Watson, <br />representatives from Teller County, Dr. Tom Wood and Mies Jane Manors were held at the mine <br />office. The main purpose of the meeting was to discuss the TRI ( TOXIC RHLHASH INVHNTORY) <br />REPORTING AND THE RECENT VISIT BY ATSDR represetatives on site and the concerns they may or <br />say not identify regarding the mining operations impact on the citizens of Victor. The visit <br />appears to have been ae a result of a letter received by EPA from one of the objectors to the <br />Amendment #7 application.A brief discussion was also held regarding the reporting of any <br />spill outside of the conainment area. The Division stated that any Cyanide or toxic material <br />epillouteide of the containment area should and must be rported to the Division. <br />Representatives from the city and the county appear to agree with such reporting <br />proceduree.The mine however dose not appear to feel the need to report a spill that is not <br />greater than what is required under HPA guidlines. The Division informed CC6V, they could <br />have the matter brought before the MLRD BOARD for some guidance on reprting spills outside <br />of the conatinment areas.
