(Page 2) <br />NINH ID / OR PROSPBCTING ID / M-80-944 <br />INSPBCTZON DATE 10/10/94 INSPBCTOR'S INITIl1LS CBl~ <br />OBSBRVATIONS <br />This was a partial inspection of the leach pad construction and associated construction <br />features of the surrounding area. <br />The liner protection cover and drainage layer placement was inspected first. Haterial that <br />is predominantly 1.5" minus material ie being dumped on already covered areas of the liner <br />and bulldozed down the very low density polyethylene (VLDPH) liner. One person was present <br />to attempt to remove oversize material during this inspection. While the placement operation <br />was being inspected, a rock that measured approximately 1• x 2' was noticed. While walking <br />over to this rock (about 150') , three other rocks that exceeded the 1.5" maximum were <br />discovered on the surface of the material and removed. <br />Soil liner placement was occurring on the north aide of the completed toe berm. One laborer <br />was on duty looking for oversize rocks in the liner material. <br />The 9360 anchor trench (lower) was inspected and a thin area of soil liner extending <br />approximately 30-35 feet along the trench was noted. The area was defined with a drill and <br />will be backfilled with more liner material as needed. <br />Also noted was an area of apparently saturated liner located above the confluence of the main <br />underdrain and the second finger drain on the left (looking up Arequa Gulch). This area <br />needs to be watched carefully prior to geomembrane placement to ensure that unsaturated liner <br />ie in place. <br />The underdrain work above the haul road fill in Arequa Gulch was inspected. There ie a <br />geologic fault zone or other anomaly that rune north-northeast to south-southwest in this <br />general area. This fault zone is conducting water to this area and that water ie running to <br />the south and outside the underdrain. This ie nn area that will be covered by waste rock to <br />make the fill for the Colorado Highway 67 relocation just above the heap leach area. These <br />springs probably do not have to be tied into the main underdrain to maintain stability of the <br />highway fill. However, tying them into the underdrain may be a good idea to lessen the <br />spring water contact with waste rock and, thus, lessen any possible contamination of the <br />water from this contact. <br />The sump above the toe berm was inspected and it was found that water was still draining into <br />the standpipe through the elite in the aides of the perforated pipe. This problem was <br />discussed during the inspection of 10-3-94 and the operator committed to placing more <br />geofabric in the pipe to aid in water filtration.
