(Page 21 <br />MINE ID q OR PROSPECTING ID # M-97-089 <br />INSPECTION DATE 10/Og/98 INSPECTOR'S INITIALS Jp HLE <br />OBSERVATIONS <br />This inspection occurred in response to two written complaints, submitted by The Friends of South <br />Montezuma Valley, Inc. DMG received the following two written complaints: <br />Complaint #1 - received by FAX on 9/10/98, and mail on 9/14/98, dated 9/9/98, from The Friends <br />of South Montezuma Valley, Susan Campbell Bell, Secretaryl'freasurer, addressing dust abatement, <br />possible deviation from the approved Mining/Reclamation Plan, possible inadequate financial <br />warranty, and requesting copies of any agreements which allow deviation from the original <br />reclamation bond and copies of any amendments to the reclamation bond. <br />Complaint #2 - received by FAX on 10/1/98, and mail on 10/2/98, dated 9/30/98, from The <br />Friends of South Montezuma Valley. The 9/30/98 complaint appeared to be focused primarily <br />towards DMG response time to the 919198 complaint, and also attempted to record selected <br />portions of a phone conversation, occurring on 9/30/98, between Mrs. Bell and DMG staff. <br />DMG responded to the complaints with an on-site inspection, which was conducted within the <br />standard 30 day response policy. During the inspection, DMG inspector did not observe any <br />serious environmental problems, or possible violations of permit conditions, the Act and Rules. <br />Operations were active during the time of this inspection. <br />Signs & Markers -Mine identification sign was posted at the mine entrance. Affected area <br />perimeter markers were observed. Public access to the permit area is controlled by locked gate <br />and fence, gate was open. Mine access road was well maintained. <br />Extraction/Processing Area - Affected lands were contained within approved areas. Portable <br />crusher with conveyors were on-site and active. Water truck and portable water tank were located <br />adjacent to the processing equipment. Operator indicated that approximately 6000 gallons/day <br />water are applied to crushing equipment and roads for dust abatement. Stockpiles of processed <br />material and overburden were located within the processing area. Vegetation debris, topsoil and <br />overburden, had been salvaged and stockpiled for reclamation purposes. Portions of the affected <br />area had been backfilled, graded and topsoil replaced, in accordance with the contemporaneous <br />reclamation requirement of the permit. Straw bales were stockpiled on site, to be utilized as mulch <br />and/or sediment control filters. Affected areas appeared stable, evidence of erosion/sedimentation <br />to affected lands and to surrounding areas were not observed. Total affected area was surveyed <br />with Brunton compass and 100' tape measure. Results of DMG field survey is illustrated in the <br />attached drawing. <br />Total disturbed area 13.2 acres <br />Reclaimed area 3.7 acres <br />Current affected area -major 8.5 acres <br />Current affected area -minor 1 acre <br />"Major" affected areas require backfill, grading, topsoil replacement and revegetation. <br />"Minor" affected areas are temporary roads and equipment parking areas which require revegetation only. <br />
