ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #; M-t99a_tnt <br />INSPECTION DATE: ~,/t sin7 INSPECTOROS INITIALS: --BK- <br />OBSERVATIONS <br />THE Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2006-2007.Presnt during the inspection was Mr. Joe <br />Gagliano the operator's consultant. <br />1) Site was permitted in 1998, for a total of 160 acres with a maximum disturbance of <br />30 acres at any given time. <br />2) Site was active at the time of the inspection. <br />3) Permit sign properly posted at the access to the property. <br />4) A scale house noted along the south side of the permit area, near the access to the <br />property. <br />5) Area had received some precipitation the last few days which was evident with some <br />ponded water at the active working face of the operation. <br />6) Large quantities of processed material scattered through out the affected area. <br />7) A small portion of the area along the South West side of the permit area has been <br />graded to a 3:1 or gentler slope. <br />8) Entire permit area is fenced. <br />9) Large quantities of what appeared to be over burden/ top soil scattered through out <br />the affected area. Operator is required to properly stockpile and protect these <br />piles. The Division has listed this as a possible problem and requires the operator <br />to properly stockpile these materials and protect them, by applying a seed mix as <br />approved in the reclamation permit, if they are mot going to be utilized with in 180 <br />days. <br />10)The maximum affected acres at any given time is 30 acres. Even though it didn't <br />appear that the operator had exceeded the approved affected acres, the Division is <br />requesting the operator to provide a survey of the TOTAL affected acres, the total <br />amount of high wall length and depth, that will be required to be sloped to a 3:1 <br />slope, location or locations of overburden/ topsoil that will be utilized for <br />reclamation's his will allow the Division to re-calculate the required financial <br />warranty. The Division requests the above information no later than 60 days from <br />receipt of this report.
