(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-teRR_nRd <br />INSPECTION DATE: s-~~-nd INSPECTOR=S INITIALS: -lD- <br />OBSERVATIONS <br />Completed a partial inspecflon of this operation to meet with a new company representative (Mr. Dwain Butler) <br />and a potential new operator (Mr. Harold Clare, A.C. Concrete f: Materials). Reviewed and discussed the <br />approved mining and reclamation plans with Mr. Clare and Mr. Butler. <br />This is a 260 acre T T2 Regular permit located about 2 miles north of Fairplay, east of Highway 9. The affected <br />land area Is limited to 79.5 acres. An adequate mine ID sign has been placed at the enfrance of the permtt <br />boundary. <br />Sand & gravel is the primary commodity for this operation. Gold is a secondary commodity. The operator <br />excavates a large pit (8 acres maximum) down to bedrock (100 foot maximum) then washes the material <br />through a sluice system. The washed material is returned to the pit so the operation is constantly being <br />reclaimed (backfilling the pit is part of the reclamatton plan). The operator is allowed to constantly move the pit <br />as long as it does not exceed 8 acres. Pit measurements were not taken during this inspection so it is not clear <br />hoHr many acres the pit currently covers. The pit is to be backfilled to within 2 feet of the natural/native ground <br />elevatton. Slope gradients are to be graded to 3:7 or less. <br />The operator crushes and screens sand and gravel material in the north part of the permit area. Large material <br />(boulder size) is segregated, stockpiled and sold. <br />Topsoil (what little there is) is stockpiled in the western part of the permtt area. <br />The main components of the reclamation plan are dirt work (dozing and grading), topsoil replacement, <br />resr:edFng (includes planting tree seedlings) and removal of all the used equipment, trash and refuse. <br />Moist of the used equipment is stored in the north central part of the affected land area. Currently, more Junk is <br />accumulating in the storage area. Since this site is not permffted as a landfill the operator must begin removing <br />all of the junk and used equipment from the site. This will be noted as a problem on page 3 of this report. The <br />operator will have 60-days to complete the corrective action. Failure to complete this corrective action, or any <br />other corrective action, could result in the issuance of a possible notice of violation. <br />While driving around the mine site only a few of the affected land boundary markers were observed. The <br />operator must install more permanent boundary markers to delineate the affected land area. This is noted as a <br />problem on page 3 of this report. The operator will have 30-days to complete the conecflve action. Affer the <br />markers have been installed, the site w111 be re-inspected to determine if all of the mining activities have <br />rensained within the 79.5 acre affected land area. <br />The financial warranty requirement will be re-calculated offer the next inspection so, ff the operator wishes to <br />keESp the bond amount at a moderate level ($259,844.00 currently), addittonal reclamatton should be <br />completed during the next 30-days (backfilling the pff, removing all the junk and unusable equipment, grading <br />the highwall in the southern portion of the affected land area and leveling all the materials on site that are not <br />going to be sold). <br />I & E Contact Address <br />NAME: Dwain Butter <br />OPERATOR: Snowstorm Sand 8 Gravel LLC <br />STREET: P (7.
