(Page 21 <br />MINE ID # OR PROSPECTING ID # M-07-038 <br />INSPECTION DATE 7/8/96 INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This partial inspection was conducted by Wally Erickson from the DMG field office in Durango, 1474 Main Avenue, Room 207, <br />Durango, CO 81301-5182, phone 1970) 247-5062, in response to request submitted by the operator for an on site meeting to <br />discuss issues to be addressed in Amendment # 3. Mr. Don Gosney and Ms. Maria Andrick met with DMG inspector, Mr. Gosney <br />accompanied the DMG inspector through out the inspection. Currently, the 112gravel operation is approved for 52.9 acres affected <br />area. DMG holds financial warranty in the amount of $44,437.00. The operator has submitted Transfer of Permit and Succession <br />of Operator, for W astern Mobile as successor operator, to DMG Denver office. DMG Denver is currently processing the request for <br />Transfer of Permit and Succession of Operator. <br />Mine identification sign was posted at the entrance to the mine site. Affected area perimeter markers were observed along the east <br />perimeter of the effected area. Operations appeared to have advanced to the approved perimeter of the permit area for the north, <br />west and south boundaries. Mining operations had not affected a small 3-4 acre area located along the east perimeter. The 3~ <br />acre area, approved as affected area but not affected by mining operations, appeared to be irrigated pasture lands which contained <br />two barns and several fence lines utilized to control livestock. All mining operations appeared to be contained within approved areas. <br />Mining and material processing activities were active in the Phase 5, 6 and 7 areas. Crushing operation was located within the <br />southern portions of Phase 6 and 7 areas. <br />Mr. Gosney indicated that an application for increase in affected area will be submitted in the near future. The application for <br />increased affected area would address implementation of Phase 4, as shown on the Master Plan, previously approved by La Plata <br />County. DMG records indicate that such request should be submitted as Amendment # 3 for the Bayfield Pit 1 operation. DMG <br />inspector discussed application requirements for Amendments with Mr. Gosney and Ms. Andrick. <br />The Phase 4 area, to be addressed through Amendment 3, is located between the office facilities area and Highway 521. Current <br />land use is pasture land. Mr. Gosney indicated that a pond would result from mining operations.
