Rollings, 370, OSM <br />Steve Shuey, Colorado Division of Minerals and Geology IDMG) <br />Bill Pecharich, SMC <br />Walt Wright, SMC <br />This inspection was conducted 5/11-12/94, however, the inspection report was <br />not written until 7/18/94. Some problems identified during the course of the <br />inspection have been addressed by DMG and SMC by this time. Ten-Day <br />Notices (TDN) will not be issued on these problems. <br />DMG permit C-81-022 was issued 10/11/93 and expires 10/11/98. The bond <br />was recalculated in TR 13 on 6/11/93 for $1,058,303. NPDES permit CO- <br />0000132 expires 10/31/96. Liability insurance is valid for the period 8/1/93 <br />through 8/1/94. The BLM lease number is C-41183. <br />Pond certifications: pond A - 12/9/92, pond B - 10/27/92, pond 8 - 7/15/87, <br />Hubbard - 8/1/84, and mine water - 11/25/91. All quarterly reports were up-to- <br />date: refuse pile, NPDES DMR's, and pond reports. <br />There were five undesigned preexisting CMP's in Elk Creek Canyon that the <br />permit did not address. DMG had SMC revise the permit to include these <br />CMP's. Four are to be permanent and one is to be removed. SMC <br />demonstrated that the permanent CMP's are oversized. DMG approved the <br />revision 7/7/94. <br />Since my last inspection, pond B has been constructed which eliminated the <br />need for many of the small ponds that used to exist around the main shop and <br />office area. These ponds have been eliminated or cut to have positive drainage. <br />The pre-law scale house location has been bermed so that is not disturbed by <br />the post-law activities. The upper storage area has post-law underground <br />locomotives, etc., stored here. <br />The permit did not have any maps showing the approved disturbed area <br />boundaries. Lack of this type of map made it difficult to determine where the <br />operator was allowed to disturb; I imagine it is also difficult to correctly <br />determine the disturbed acreage for bonding purposes. DMG had SMC submit a <br />
