(Page 2) <br />MINE ID # OR PROSPECTING ID #: M_taat_t3~____ <br />INSPECTION DATE: t010N05 INSPECTOR=S INITIALS: A IW <br />OBSERVATIONS <br />This was a normal monitoring inspection conducted by Tony Waldron of DMG. The operator was contacted but <br />did not accompany the inspector during this inspection. The site is located approximately 20 miles south and 1 <br />mile east of Walsh, Colorado. The pit is situated in an upland area of gently rolling terrain. <br />There was a mine identification sign present at the entrance to the mine site and the affected area was clearly <br />delineated. There is a significant area (20-40 acres) of historically disturbed and un-reclaimed area to the east <br />of this pit. In addition, there seems to be some confusion about the exact location of the permit boundaries for <br />this pit. Originally, the permit area was sort of an irregular diamond shape encompassing approximately 9.4 <br />acres as illustrated on the enclosed map (labeled map A). However, at some point the boundaries were <br />"revised" to their current configuration of a general rectangular shape also encompassing approximately 9.5 <br />acres as shown on the annual report map (labeled map B). In addition, the most recent annual report map <br />identifies a "New Disturbance" area to the south and east of the existing permitted area. Apparently, this was <br />going to submitted as a new 11 Oc application, however, side-by-side 110's are no longer allowed, so in order to <br />incorporate this area fhe existing pit would need to be converted into a I I2c permit. Also, in reviewing the <br />permit file, the inspector became aware that this file does not contain a mining plan map or a reclamation <br />plan map that accurately reflects the existing situation at this site. Therefore, this is being cited as a problem on <br />page three of this report along with corrective actions and correction dates. Finally, it was also observed that <br />this file does not contain a Performance Warranty, therefore, a blank Performance Warranty form has been <br />enclosed for you to complete and return. This is also being cited as a problem on page three. <br />No other problems were noted and this concluded the inspection. <br />18.
