ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MIfJE ID # OR PROSPECTING ID #: ti+-IeRn_~ed <br />INSPECTION DATE: 6/»/ed INSPECTOROS INITIALS: ~~ <br />OBSERVATIONS <br />Th.e Division conducted a weekly construction progress and monitoring <br />inspection of the site on 6/17/04. Present during the site inspection were <br />Phil Barnes and Mark Tidquist of CC&V. <br />Inspected the additional lined spill prevention area near the ARD building. <br />It was found to be in good repair. Continued the inspection to the Phase <br />IVC, construction area. Drain cover was being deployed at the South West <br />portion of the expansion area. Most of the West side of the expansion areas <br />had both clay and geomembrane liner in place. The deployed liners were <br />walked and inspected no problem noted. <br />Artchor trench along the lower portion of the North Facing slopes is <br />pairtially in place. Golder QA/QC personnel were on site overlooking the <br />deployment. Inspected the Phase I, II, III AND IV pregnant solution <br />collection areas and recorded the following readings from the High and Low <br />Volume solution collection systems. <br />Phase I High volume solution collection transducer # 1 was recorded at22.7 <br /># 2 was at 25.8 and # 3 was at 24.7. Low volume transducer # 1 was recorded <br />at: .15 and # 2 was at .56. Piezometer for the pond was at 34 feet. <br />Phase II AND III High volume solution collection transducer # 4 was <br />recorded at 30.8 #5 was at 26.7 and # 6 was at 31.8 and # 26.7 and # 6 was <br />at: 31.8. Piezometer for the pond was at 31.9 feet. <br />Phase IV Low Volume solution collection transducer # 1 was recorded at 18.2 <br />and # 2 was at 8.9 (inches of hydrostatic head) <br />Hlgh volume solution collection transducer # 7 was recorded at 22.8 # 8 was <br />at: 21.9 # 9 was at 23.2 and # 10 was at 23.6. 9 ALL TRANSDUCER READINGS <br />UfJLESS NOTED ARE IN FEET OF HYDROSTATIC HEAD). <br />Inspected the external pond, it was holding some water. Low volume <br />transducer #1 was recorded at 13.1 and # 2 was at 10.4 inches of <br />hydrostatic head. <br />There was some drilling on the pads, being conducted at the time of the <br />inspection, to determine ore leachability potential. <br />Inspected the main Cresson area, storm water management sumps along the <br />h<~ul road appeared to be in good repair. Inspected the East Cresson mining <br />area, it was active at the time of the inspection, both backfilling and <br />mining are being conducted contemporaneously. <br />Inspected the under drain area, all lower ponds were dry. Both the South <br />and four-inch pipes were discharging. <br />
