• (Page 2) • <br /> MrtNE ID # OR PROSPECTING ID # M-78-208 <br /> INSPECTION DATE 5/25/99 INSPECTOR'S INITIALS CLK <br /> OBSERVATIONS <br /> This inspection of this 9. 5 acres 110 site was done as a part of the routine monitoring <br /> program. <br /> 1) Signs and boundary markers are in place as required by the Rules (metal posts and road <br /> boundaries) . <br /> 2) Approximately half of the site, the eastern half, has been designated as Jefferson County <br /> open space, and has not been disturbed. <br /> 3) Positive drainage off the site is occurring, with little pooling of water in front of the <br /> drainage culverts. However, the `retention pond' area --- the bermed area on the north side <br /> --- needs to be re-established and mucked out. (PHI) <br /> 4) Two levels of benching have been established, with the upper level complete and <br /> revegetated (trees growing, grass established) . The lower bench is the appropriate width <br /> required in the permit (15' -25' ) and appears to be stable (no cracks, fissures, or erosion <br /> noticed) . The slopes down from the benches appears to be 0.5H: 1V (as required in the permit) <br /> on the upper bench, and 1 .5H:1V on the lower bench. <br /> 5) The maintenance shop and equipment storage are still onsite; some trash was observed on <br /> the site, such as loose piping and an abandoned truck cab. <br /> The Operator and the new Landowner were present during the inspection of this site, and <br /> explained that Mr. Patterson wishes to take over the permit from Mr. Nelson. The Succession <br /> of Operator process was explained, as well as the probable necessity for a new bond. Upon the <br /> receipt of the SO application, DMG will recalculated the bond based upon this inspection's <br /> observations (assuming that the application is received within 60 days) . <br /> A request for a partial release of the County-designated free space area was recommended for <br /> after the transfer of the permit was completed. <br /> No other problems were noted at this time. <br /> I & E Contact Address cc: Carl B.
