(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-iaan-~aa <br />INSPECTION DATE: »-~'~-n~ INSPECTORS INITIALS: ___6LC- <br />The Division conducted a monitoring and construction progress inspection of the site on 12-13-O1. Present <br />during the inspection were, Mark Tidquist and Phil Barnes of CC&V. A brief discussion, with both Golder and <br />CC&V engineering staff was held regarding the minor changes to the specifications for Amendment # 8. <br />They were informed again that as a clarification to the permit a small write up of the changes should be <br />submitted to the Division, which they agreed to oblige. <br />The physical inspection began around the Phase II pregnant solution collection systems area, where the <br />following readings were recorded from the High and Low volume solution collection system transducers. <br />High volume pump # 4 was recorded at 19.4, # 5 was recorded at 26.5 and # 6 was recorded at 31.0 <br />Low volume pump # 1 was recorded at .14 and # 2 was at .27 (ALL READINGS ARE IN FEET OF HEAD) <br />Inspected the external pond. It was holding some water, which appeared to be frozen. The low volume <br />transducer was recorded at 16.6 inches or 1.3 feet of head. However, based on the amount of water that <br />was frozen in the main pond, the hydrostatic head, recorded at the low volume transducer gauge, might be <br />off. Mr Barnes agreed to look into it and notify the Division as soon as possible. Since the pump back system <br />is not in place, no cyanide solution can be stored in the pond. Mr. Tidquist stated that the pump back system <br />might not be in place until next March. <br />Inspected the UNDERGROUND REMIDATION AREAS OF Phase IV. Work was continuing in identifying thee <br />areas. As was the case in Amendment # 7, the operator is given the latitude to change the remediation <br />specifications, as long as the Division is given notices of the changes in advance, for our records and <br />evaluation. No major problem was noted during the inspection. All the minor specification changes <br />requested by the operator were in line with what was done during Amendment # 7, Phase II AND III AREA <br />remediations. <br />Inspected the new crusher area for Phase IV, construction work was continuing, the hill fickler wall <br />appeared to be completed.
