~ (Page 21 "'~ <br />MINE ID # OR PROSPECTING ID k M-95-057 <br />INSPECTION DATE 06/06/97 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of 1 10 permits. The permittee was not contacted about the <br />inspection, but contact was made with the permittee's designated "inspection contact", Warren Wilcox of Gunnison Gravel, who <br />could not be present at the inspection. There were two people present onsite during the inspection, one of whom is named on page <br />one of this report. <br />The required permit ID sign was posted at the entrance, and the permit boundary corners were marked by survey pins. The operator <br />is aware of the locations of the boundary, but additional installation of more visible markers might be considered. At present though <br />this topic is not a problem. <br />The site is active, with the following activities observed: topsoil stockpile movement along the north boundary, screen equipment <br />being set up for use onsite, and preparation for mining the bank material in the northwest end. The pad is level, and presumably <br />will not be mined down lower. The pad is bermed along the southwest side. Stockpiled processed material included primarily 3/4 <br />inch gravel, plus minor amounts of 3 inch rock and salt sand. There were large amounts of raw material to process also stockpiled <br />at the toe of the NW knob to be mined. <br />The newly installed screen operation included a generator trailer and 300-gallon fuel tank on skids. It appears that there has already <br />been some diesel spillage at the fueling location. There is the potential for additional spillage, plus potential for uncontained leakage <br />because there is no seconary containment structure present. The topic of "acid or toxic materials" is noted as a problem in this <br />report due to the observed soil contamination and lack of containment structure The operator must correct this problem by the date <br />shown on the last page. Recommended correction might include complete removal and proper disposal of all contaminated soil, or <br />excavation and spreading over the site of all contaminated soil, plus installation of a lined and bermed fuel storage pad. (Please call <br />to discuss if questions.) <br />At the east end of the operation pad are several items, including a storage trailer, miscellaneous spare parts and materials, and at <br />least eleven 55-gallon drums containing mostly waste oil. There is very obvious spillage of oil on the ground and no containment <br />under or around these vessels. This is also a problem under the same topic as the oroblem noted above. The operator must clean <br />up the contamination and either remove these drums from the site or provide adequate containment. See the last page for the <br />corrective action. <br />The operator should note that for all items which must be removed from the site and/or any necessary clean up, the cost of <br />performing these tasks must be covered under the bond. Presently the bond covers only earthwork. If the site is not cleaned up <br />or if future storage of these materials is to occur (there is nothing wrong with such storage if they are safely and fully contained) <br />then the bond must be adjusted accordingly. Until this office receives evidence of such clean up as described on the last page, the <br />bond will not be recalculated nor be considered insufficient to perform all reclamation. At present, the "financial warranty" topic <br />is not noted as a problem. <br />There were no other items noted during the inspection, nor other problems.
