~- <br />• (Page 2) • <br />MINE ID # OR PROSPECTING ID / M-80-244 <br />INSPECTION DATE 12/11/94 INSPECTOR'S INITIALS GSC <br />This was a monitoring inspection of the Phase 1 leach pad construction. Dave Dix and Terry <br />Mandziak were on-site. Placement of drain cover fill was complete on the portions of the pad <br />to be certified. Construction crews were employing Cat D-9 dozers and hydraulic excavators <br />to construct the upper Arequa Gulch temporary berm and Wilson Gulch dam and diversion <br />channel. The diversion channel and parallel road were near completion along the southeastern <br />elope. The road was being surveyed to elope away from the pad according to Dave Dix. A <br />front end loader was pushing fill in the upper Arequa Gulch sump area. Golder QC monitors <br />were observed on site. <br />Riser pipes were observed in place at the pregnant solution sump. The base of the pipes were <br />covered with granite fill to a level about 3 feet below the top of the perforations. <br />The diversion channel area north of Highway 67 was inspected. A portion of the diversion <br />ditch above the intersection of Reaches A and B exceeds a 2:1 elope. The Division verified <br />that this steeper portion of the channel had been recently grouted. <br />The diversion channel culvert had been placed across Highway 67, west of the office and mill <br />building. The frame for the southern end of the culvert had been recently cemented into <br />place. A hydraulic excavator was lining the diversion channel south of Highway 67 with rip <br />rap. This portion of the ditch had been excavated adjacent to a stabilized topsoil <br />stockpile. The western portion of the stockpile had been cut to a grade of approximately <br />1.5:1 to 2.5:1. The Division observed and addressed this issue in it•s 11-30-94 inspection <br />report which stated that the operator must further assure that the topsoil pile is protected <br />form erosion or contamination caused by diversion ditch installation or operation. The <br />exposed elope will need to be stabilized. This can be accomplished by cutting back the elope <br />to 3:1, temporarily stabilizing with hay bales during the winter and reseeding the elope with <br />approved seed mix during the spring.
