<br />Larry Oehler <br />November 11, 1993 <br />Page 5 <br />Corrective action item 4 states: <br />'~An engineering technician (John) of SRK stated that <br />spills similar to the one on 10/1/93 had occurred. <br />Please provide a report to the Division on the sF~ills <br />referred to above. This can be done in conjunction <br />with your response to item two.~~ <br />We are unaware of any similar spills. John DeAngelis of SRK <br />has prepared an explanation of his remarks to the Division <br />(see attached). As mentioned in the memo by John DeAngelis <br />on September 30, 1993, prior to the Division's arrival. at <br />the site, the PVC dewatering line was relocated in <br />preparation for the liner installation. This line carries <br />uncontaminated groundwater being pumped from the area of the <br />West Pit to the tailings disposal area;the water level is <br />being lowered so that mining activities do not intersect <br />groundwater. The pit dewatering pump was turned off, and <br />the line was believed to be drained. A small amount of <br />water (estimated to be less than 300 gallons) remained at a <br />low point in the line. When the pipe was disconnectec', at <br />the PVC joint, the groundwater flowed onto a portion of the <br />compacted soil liner. The saturated portion of the soil <br />liner was removed and replaced with new material. <br />Again we request that all issues of concern be raised with <br />BMRI during the inspection or at the close-out meeting to <br />maintain effective and efficient communication. We further sucfgest W;fG 6~ Ft <br />that in order to avoid the inclusion of incorrect date. in tR K <br />the inspection reports that the Division's report be <br />completed during the site visit, and the report issues. at O ~. <br />the close-out meeting.
