Corrective Action Date: Mav 5, 1992 <br />Topic: SF (Processing Facilities) <br />PB (Posa ib le Violation) <br />The AVR appears to have been underpowered. Air delivery may be as mLCh as 5 times lower <br />than the design specifications, and the residence time is too short. <br />The operator should examine the AVR system and determine why i:he system failed. <br />Measures to mitigate the problem should be taken before retur:iing the plant to <br />operation. <br />3. Corrective Action Date: Mav 8. 1992 <br />Topic: FW(Fish & Wildlife) <br />PB(Problem) <br />Two ducks found in a tailings pond may have died of cyanide poisoning. <br />The operator should mail the the Division the results of the necrop:3y as soon as those <br />results are available. <br />4.
