(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-i os{n_~de <br />INSPECTION DATE: u-~R-n~ INSPECTOROS INITIALS: -131L <br />08SERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the Phase I, II, III AND IV areas of <br />the leach pad and water sampling from the North Under drain Discharge, on 8-21-03. Present during the <br />lnspectlon were Mark Tidquist and Phil Barnes of CCBV and Lawrence Meyers, for the water-sampling portion of <br />the inspection. The physical lnspectlon began around the Phase IV pregnant solution collection areas. <br />While In the area collected the following readings from the Hlgh and Low Volume Solution Collection <br />Transducers for Phase IV. High Volume Transducer # 1 was not functioning at the time of the inspection, the <br />pump was on, and as noted in the previous inspection reports, once all the power transfer is complete, it should <br />be functioning as designed # 2 was at 22.5 feet of hydrostatic head. Low Volume transducer # 1 was at 5.7 and <br /># 2 was at 3.7 (all readings are in inches of hydrostatic head) both pumps were on at the time of the inspection. <br />No major problem noted. Inspected the leach pad In the Phase IV PSSA area, some ponding OF WATER WAS <br />STILL NOTED, more like SMALL puddles of solution, AS REPORTED IN THE PREVIOUS INSPECTION REPORT, a D-6 <br />DOZER was ON S17E TO REWORK THE UPPER LAYER, to allow percolation, no major problem noted. <br />Inspected the Phase I AND II Pregnant solution collection areas and recorded the following readings from the <br />High and Low volume solution collection areas. <br />Phase I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS recorded at 39.5# 2 was at 41.9 and # 3 was <br />at 41.6. Low volume transducer # 1 was recorded at .22 # 2 was at .32. <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 36.8 # 2 WAS AT 33.2 AND #6 <br />WAS AT 38.4. LOW VOLUME TRANSDUCER # 1 WAS AT .17 AND # WAS AT .38 (ALL READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD) Plezometer for the pond was recorded of 34.1 feet. <br />Inspected the external pond, it was holding water from the recent storms, while In the are recorded the following <br />readings from the Low Volume solution collection transducers # 1 was at 7.7 and # 2 was at 9.5 readings are in <br />Inches of hydrostatic head. Earth moving work in anticlpatlon of liner deployment, below the ARD area for the <br />additional containment is continuing, a portion of the area has been lined with clay in antlctpation of <br />geomembrane liner deployment <br />Inspected the under drain area. Both the 4-inch pipe, South and North under drains were discharging at the <br />time of the inspection. The flow was being pumped back onto the leach pad. All three lower ponds were still <br />holding a lot of water; the water was being pumped back onto the pad at the time of the inspection. With <br />Lawrence Meyers present, an eight-ounce sample for Cyanide analysis using CC&V bottle was collected. The <br />sample was placed on ice and delivered to the CDOHBE lab that same afternoon. All the lower ponds and <br />surface water diversion structures were also holding water but were not discharging at the time of the <br />inspection.
