Rollings, 370, OSM <br />Tony Waldron, CO Division of Minerals and Geology IDMG) <br />Jim Kiger, CCC <br />Juan Garcia, CCC <br />This inspection started as a complete oversight inspection, however, observed <br />site conditions have required me to request technical assistance from the <br />Western Support Center. This inspection report will address all conditions other <br />than the diversions used to route runoff to the siltation structures. The <br />conditions that warranted my request for technical assistance will not be a <br />subject of this report. These conditions include the design, location, <br />construction, and maintenance of the ditches, and the offsite areas impacted by <br />ditch failures. An additional inspection report will be written after completion of <br />the technical review. <br />The DMG permit, C- - 1,9,,,was issued 11/5/92 and expires 11/5/97. The <br />DMG issued one violation as a result of this inspection. C-94-013 was issued <br />for not passing all disturbed area runoff through a siltation structure. The cited <br />area was being stripped of topsoil during the inspection. CCC had failed to <br />construct the sediment control facilities for this location prior to disturbing the <br />area. I believe the violation was terminated the next day. <br />permit is for acr ,with an approved disturbance acreage of 3810 <br />acres. The NPDES permit expires 6/30/97. Liability insurance was valid for the <br />period 6/30/93 through 6/30/94. The posted bond amount is for 58,747,405. <br />Pond and fill certifications and inspection reports were current. <br />Two locations of the Streeter fill ditch were repaired during the inspection. The <br />first is a location in the ditch where riprap has washed from the inside edge of <br />the ditch, and 20 feet below this spot, from the outside edge of the ditch. CCC <br />used a dozer to regrade the riprap and cover the exposed underlying soil. The <br />second location is a spot where the ditch had been breached on the outside <br />edge, runoff swept around outside the ditch, and then reentered the ditch at a <br />lower point. The area outside of the ditch had been covered with a sediment <br />delta. This area was also repaired by CCC. <br />
