• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-80-136 <br />INSPECTION DATE 01/12/00 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were recorded during a routine inspection of [his 149 acre 112 site., <br />1. Signs were posted at the entrance to the pit, and boundaries were marked, as required by the Rules. <br />2. The overburden berm along the river is still in place, and the 75' setback was still being observed. <br />3. Measurements were taken of the smaller topsoil stockpile along the north side of the permit area. It is approximately 54 paces <br />long (81'), with side slopes upward of 40 paces (60'), with a height and width of approximately ZO', for a total volume of <br />approximately 1776 CY. Another, much larger stockpile (approximately 5' shorter than [he nearest telephone pole) will be <br />measured from an aerial photograph taken in September 1999. This photo (along with ascale) is to be submitted by the <br />Operator by February 14, ?000, so that the bond may be recalculated accurately. (PBI) <br />4. The concnae plant and screens were confirmed as portable entities; however, the building next to them is not. Measurements <br />were taken of the aluminum building and the concrete block beneath it, in order for the cost of removing them to be calculated <br />into the bond. The conctti=te slab is 6" thick with no rebar, measuring 100'x75', and the building is 45'x70', with a height of <br />approximately I5'. The concrete slab under the soil silo is I5'x15'x3', and does contain rebar. <br />i <br />5. Area A of the site has been completely reclaimed to a reclamation and road, but it does not conform with the current <br />reclamation plan. A Technical Revision to the pernit must be submitted if these current contours and landscaping are to be <br />the final, including a map of the new area and approval from the County for this land use. , <br />6. Some of the permit areas which are now part of the local animal shelter, and the newly reclaimed Area A, may be eligible for <br />partial release in [he spring, since they have been almost completely reclaimed and there are no plans to re-excavate those <br />portions of the site. <br />No other problems were noted at this time. <br />the proposed reclamation plan. <br />I & E Contact Address <br />NAME Phil Lona <br />OPERATOR Lafarge/ W.
