_~ . <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-80.244 <br />INSPECTION DATE: 6-10-00 INSPECTOR'S INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the area on 6-10.00. Present during the <br />inspection was, Scott Lewis of CC&V. The inspection began in the Phase III area. Both clay and geomebrane liners were <br />being deployed at the time of the inspection. Approximately half of the phase III ore storage area had already been <br />deployed with geomembarane liner. The deployed liner was walked and inspected, no major problem noted. A portion of <br />the clay lined area, east facing slopes showed some rutting, which the Golder QAIQC assured that prior to geomembrane <br />liner deployment, the area will be reworked to remove the rut. Due to dry spring, water was being applied on to the clay <br />liner using water trucks to make sure the optimum moisture is achieved prior to compaction. According to the operator, the <br />Phase III area construction should end with in three to four weeks. <br />Inspected the Phase II pregnant solution area and collected the following readings from the high volume and low volume <br />solution collection system transducers. <br />Low Volume pump #1 read .42 feet of head <br />#2 read .20 feet of head <br />High Volume pump #4 read 22.2 feet of head <br />#5 read 27.2 feet of head <br />#6 read 25.6 feet of head <br />Inspected the East Cresson pit area, it was active at the time of the inspection, no major problem noted. <br />With Ron Roberts of CC&V and Bob Nickels of Golder present, the operator requested a change in geomembrane <br />deployment for the Phase II dam raised area from perpendicular to horizontal deployment. Since this was a change from the <br />approved plan under Amendment # 7, operator was notified to submit the change in writing and that the Division would <br />not have a problem in approving the request as long as the change does not compromise the integrity of the weld.
