The site is a large 1 12 operation covering 314 acres, with all of the acreage to be affected. Present during
the inspection was Mr. Mike Macdonald of Kiewit Construction. According to Mike, Large is going to be
operating the site by utilizing a conveyer system from their adjacent operation and that all mining will be
conducted by Lafarge in the Kiewit permit. Mike was notified that even though Lafarge will be operating as far
as the Division is concerned, unless the Lafarge succeeds the permit, all responsibility for the permit # M-1987-
171, WILL BE THE RESPONSIBILITY OF KEWITT CONSTRUCTION.
The entire site was driven and inspected, site was not active at the time of the inspection. When site was
inspected in 2001, as part of pre-operational inspection to add approximately 2 acres to the permit, which
included a paved access road, no problem was noted with the active portion of the permit area. Permit area is
all fenced.
Inspected the storm water detention pond along the south side of the permit area, it was found to be in good
repair. Given the fact that Lafarge is going to be actively mining the area, and since the exiting warranty is only
$138,000.00 for maximum phased affected acreage of 50 acres, the Division request the operator to submit a
complete survey of the affected area including the dimensions for the Temporary/shop building that is already
in place since the Amendment was approved in 2001. The dimensions for the shop must include the type,
depth of footings be it concrete or other wise, so the Division will calculate the required warranty prior to afull-
scale operation being conducted by Lafarge. Operator agreed to provide all the requested information to
allow the Division to calculate the needed warranty no later than 6/19/06.
Inspected the pit floor area, pit high walls were stable, no sign of erosion features noted.
Topsoil stockpiles along the East side of the access road were properly protected. No major sign of weed
infestation noted.
Once the Division receives the needed information as requested, the warranty will be calculated and the
operator will have 60 days to post the added warranty. According to the amendment the paved access road
was approved to remain as a permanent structure to allow access to the property after mining ceases in the
area. Permit sign was properly posted ai the access to the property.
I & E Contact Address
NAME: TONY COLLLINS
OPERATOR: KIEWET CONSTRUCTION
STREET: 7996 SnI1TH PI ATT C'ANYnN Rr~Af)
CITY/STATE/ZIP: I ITTI Frr~N c~ Rnt 9R
CC:
t BL
r FS
I' H W
t HMWMD (CH)
I'
r SE
r WOCD (CH)
I' OTHER
cc: I' CE
