(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2000-113 <br />INSPECTION DATE 3/5/03 INSPECTOR'S INITIALS GRM <br />OBSERVATIONS <br />This inspection was preformed at the request of the proposed Succeeding Operator. Mr. Rod Johnson and Darrell Ewing from <br />Lafarge North America met the DMG Environmental Specialist at the site. The purpose of the meeting was to review startup <br />activities to ensure the approve Mining Plan is initiated. Also there were a few issues that needed clarification that will be <br />discussed below. <br />At this time no disturbance on site is noted. Surveyors were observed on site working. Permit boundaries forthe first stage of <br />mining in the West Property Mining Area were set. The crewwas establishing berm locations and outlet locations at the time. <br />All points observed appeared to correspond to those proposed for this Mining Area. <br />The requirements of the use of geo-membrane fabric in the construction of the river barrier berms were discussed. Staff will <br />review the requirements and forward any information and clarifications to the operator with this report. <br />The Succession of Operator was also discussed. It is important that this documentation been submitted at the earliest time <br />possible. <br />Since the operation is preparing for the initiation of disturbance, Staff requests that the mine identification sign be installed per <br />the requirements of Rule 3:1.12 (1) that state: The operator shall post a sign at the entrance to the mine site which shall be <br />clearly visible from the access road and specify the following; a) the name of the operator; b) indicate that a reclamation permit <br />for the operation has been issued by the Colorado Mined Land Reclamation Board, and; c) the permit number. <br />No other issues were noted or discussed.
