., ~ <br /> <br />' MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 11-23-99 <br />(Page 2) <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS HK <br />The Division conducted, a monitoring inspection of the Bite on 11-23-99. Present during the <br />inspection wee, Mr. Scott Lewis of CCBV. The main purpose of the inspection was to see the <br />construction progress of the Phase two Toe DAM. Under the approved Amendment #7, the Toe Dam <br />was supposed to be raised by 5 feet, which was later through a Technical Revision the <br />operator requested to raise the dam by seven feet rather than five feet as approved in the <br />Amendment. Inspected the Toe Dam area, due to Holiday and some adverse weather conditions, <br />no construction activity was taking place at the time of the inspection. While in the area, <br />the following readings were recorded from the High and Low volume solution collection eyatem <br />transducers for phase II pregnant solution collection area. HIGH VOLIIMH PUMP #4 HAD 20.3 <br />FHHT OH HEAD, PVMP #5 HAD 21.0 FEET OF HEAD AND Pi1MP #6 HAD 25.8 FHHT OF HEAD. In addition <br />to these transducers, the operator has placed four PVC pipe piezometer with in the pregnant <br />solution collection eyatem for added safety so the head on the system can be measured with <br />an electronic meter. The following readings were taken from the low volume solution <br />collection transducers. Pump # 1 had .43 feet of head and Pump #2 had .20 feet of head. Flow <br />was measured at 4.0 gallons g.p.m. Inspected the main Cresson Pit area, it was active at the <br />time of the inspection, no major problem noted. The North Cresson pit area was also <br />inspected, it was also active at the time of the inspection. A brief discussion was held with <br />Mr. Lewis regarding the vibrating wire piezometer that are in place in both the Phase I had <br />II ore storage areas. The reason these vibrating wire piezometer were required under <br />Amendment # 6 was to measure the head in the double liner eyatem of the ore storage area. <br />However, these piezometer have never functioned as expected and the Division has not required <br />the recorded readings since phase II was completed. Since the Division does not have the <br />calibration instrument for taking readings, and the fact that if there is a failure in the <br />eyatem the likeliest place that the Division will see exceedance in cyanide and PH will be <br />in the under drain. The Division has never considered these vibrating wire piezometer to be <br />a vital part of the monitoring program for the site, However, since these piezometers have <br />to be extended to the phase II area, there is no point in extending them if they are not <br />going to function in the long term. Therefore, the Division requests CCBV to provide another <br />design or designs that ie capable of measuring the head in the ore storage area prior to <br />extending the present system into the Phase III area ae approved in Amendment #7.
