OBSERVATIONS
The Division conducted a monitoring and construction progress inspection of the site on 8-22-02. Prior to
the physical inspection of the site, a brief discussion regarding the pending engineering certification report
for the Phase IV (a) leach pad facility, was held with Phil Barnes. He pointed out on the map, the areas to
be covered in the certification report, it was as noted in the field.
The physical inspection began around the inter face between Phase I and Phase IV (a) leach pad AREAS.
Terry explained the sequence of drain cover placement. One D-9 and one D-6 dozer were pushing drain
cover at the time of the inspection. GOLDER's QA/QC were present over looking the deployment. According
to Terry, the end of this week or early part of next week, drain cover deployment, should be completed. No
major problem noted.
Continued the inspection to the Phase III AND PHASE IV (b) inter face area. Back fill'and grading work was
continuing. The small area that was required to be lined along that area had partial deployment of
geomembarne liner in place. All the upland diversion for the area are completed, and in place. The leak
detection trench was being lined at the time of the inspection. No major problem noted.
Discussed briefly about the underground remidiation plan, for the PHASE IV, PSSA area. Terry explained
that most of the back filling was completed and that as mentioned in the previous report, the workings
were found to be shallower than first thought and that the remidation was conducted as approved by the
Division.
The old LOB that was in the area, has been dismantled, and will be removed from site in anticipation of the
pad expansion .The new LOB AND CRUSHER ARE NOW FULLY FUNCTIONAL.
Inspected the external pond, it was holding same rain water. While in the area, collected the following
readings from the Low volume solution collection transducers. # i WAS RECORDED AT 12.5 NAD # 2 WAS
AT 9.6 (READINGS ARE IN INCHES OF HYDROSTATIC HEAD)
Inspected the phase II pregnant solution collection area and recorded the following readings from the
Phase HIGH AND LOW VOLUME solution collection transducers.
High volume collection Transducer # 4 was recorded at 23.5 # 65 was at 18.7 and # 6 was at 23 .4
Low volume transducer # i was at .17 and # 2 was at .16 (all readings are in feet of hydrostatic head )
Piezometer for the area was recorded at Z8.8 feet.
Inspected the under darin area, work on the pump back sytem was continuing. Both the 4 inch pipe and
south under drain were discharging, north under drain was still dry. No problem noted.
With Amy Meketi present drove to the Carlton Tunnel Discharge area. All the ponds were holding water and
the system was discharging to Four Mile Creek, the discharge has an NPDES PERMIT FROM CDOHScE. The
entire area was walked AND INSPECTED, POND EMBANKMNETS WERE FOUND TO BE IN GOOD REPAIR, NO
MAJOR PROBLEM NOTED.
