MINE ID # OR PROSPECTING ID #: M-198n-~aa <br />INSPECTION DATE: a-»-n~ INSPECTOR'S INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on 8-22-02. Prior to <br />the physical inspection of the site, a brief discussion regarding the pending engineering certification report <br />for the Phase IV (a) leach pad facility, was held with Phil Barnes. He pointed out on the map, the areas to <br />be covered in the certification report, it was as noted in the field. <br />The physical inspection began around the inter face between Phase I and Phase IV (a) leach pad AREAS. <br />Terry explained the sequence of drain cover placement. One D-9 and one D-6 dozer were pushing drain <br />cover at the time of the inspection. GOLDER's QA/QC were present over looking the deployment. According <br />to Terry, the end of this week or early part of next week, drain cover deployment, should be completed. No <br />major problem noted. <br />Continued the inspection to the Phase III AND PHASE IV (b) inter face area. Back fill'and grading work was <br />continuing. The small area that was required to be lined along that area had partial deployment of <br />geomembarne liner in place. All the upland diversion for the area are completed, and in place. The leak <br />detection trench was being lined at the time of the inspection. No major problem noted. <br />Discussed briefly about the underground remidiation plan, for the PHASE IV, PSSA area. Terry explained <br />that most of the back filling was completed and that as mentioned in the previous report, the workings <br />were found to be shallower than first thought and that the remidation was conducted as approved by the <br />Division. <br />The old LOB that was in the area, has been dismantled, and will be removed from site in anticipation of the <br />pad expansion .The new LOB AND CRUSHER ARE NOW FULLY FUNCTIONAL. <br />Inspected the external pond, it was holding same rain water. While in the area, collected the following <br />readings from the Low volume solution collection transducers. # i WAS RECORDED AT 12.5 NAD # 2 WAS <br />AT 9.6 (READINGS ARE IN INCHES OF HYDROSTATIC HEAD) <br />Inspected the phase II pregnant solution collection area and recorded the following readings from the <br />Phase HIGH AND LOW VOLUME solution collection transducers. <br />High volume collection Transducer # 4 was recorded at 23.5 # 65 was at 18.7 and # 6 was at 23 .4 <br />Low volume transducer # i was at .17 and # 2 was at .16 (all readings are in feet of hydrostatic head ) <br />Piezometer for the area was recorded at Z8.8 feet. <br />Inspected the under darin area, work on the pump back sytem was continuing. Both the 4 inch pipe and <br />south under drain were discharging, north under drain was still dry. No problem noted. <br />With Amy Meketi present drove to the Carlton Tunnel Discharge area. All the ponds were holding water and <br />the system was discharging to Four Mile Creek, the discharge has an NPDES PERMIT FROM CDOHScE.
