(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-t9RR_t t~ <br />INSPECTION DATE: 7-~t-nd INSPECTOR=S INITIALS: __~O__ <br />OBSERVATIONS <br />Completed a partial inspection of this operation to introduce Mr. Tony Waldron, Environmental Protection <br />Specialist, to the operators and to the reclamation and cleanup operations that are occurring at the site since <br />Mr. Waldron is about to become lead speclaRst of the site. <br />Reviewed most of the permM area (some from a distance). Discussed current activities at the site; recently <br />seeded areas of the access road, the mill building area (removal of the primary crusher, sag mill etc.), <br />reclamation progress of the tailings impoundment, and the seep area on the bank of the Rito Seco. <br />Toured the R/O unit, which continues to operate a few days a week. In order to maintain the required <br />groundwater elevation in the backfill material, the operator continuously pumps two of the monitoring wells in <br />the west pit. When the R/O unit is not running the water is pumped to the lined pond north of the facilNy (pond <br />#1). When the unit is operating, water is pumped from pond number 1 and the capture wells. CurrenHy, about <br />790 GPM needs to be extracted from the West pit aqu'tferto maintain fhe required water level. <br />Looked at the old heap leach area, the collection pond, the toe berm, the stormwater diversion on the east side <br />of the tailings impoundment and the stormwater diversion system on the west side of the west pit, the equipment <br />parking area and the waste dumps. <br />Side castings of the access road have been pulled back and leveled. The area was topsoiled and seeded a <br />couple of years ago (Inter-seeded last year). Vegetative growth looks strong. <br />The tailings Impoundment pond has finally dried up. It has been dry for about 3 weeks. R/O sludge is still being <br />deposited on the southeast corner of the pond. The material is placed in an excavated pit and covered with <br />overburden and topsoil. Vegetation in the impoundment looks excellent.
