ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_I9s3n_~da <br />INSPECTION DATE: 6/y'~~ 7 INSPECTOROS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a partial inspection of the site on 6/23/06, while in the area at <br />the request of the operator to see the proposed location for temporary placement of <br />additional carbon columns. Present during the inspection were Tim Comer, Gary Goodrich & <br />Denny Demmer. <br />CC&V requested a site visit to show the proposed location of the additional carbon columns <br />that they plan to add to increase the Gold production. Mr. Dimmer pointed out the location <br />along the south side of the ARD building, in the certified lined area o the Phase I leach <br />pad. He also explained that the total pumping capacity of these columns will nit exceed <br />700 gpm, CC&V is approved to pump a total of 17,000, gpm daily maximum. At present <br />according to MR. Dimmer, the mine is at 14,000gpm. The Division agreed with the location <br />and requested a Technical Revision to the permit to add the columns which they agreed. <br />Inspected the Phase I, II, II&IV Leach pad areas and collected the following readings from <br />the High and Low Volume Solution Collection Transducers. According to Tim Commer, due to a <br />transformer failure, both the low volume solution collection pump system for Phase I &II <br />WERE RUNNING ON AUXILARY GENERATOR located at the North End of the External pond compound. <br />Mr. Commer stated that at no time did the hydrostatic head exceed the 2 feet maximum <br />allowable limit. <br />Phase I High volume Solution Collection Transducer #1 was recorded at 7.0 (read out meter <br />still not functioning) #2 was at 32.7 and # 3 was at 33.8 piezometer for the pond was at <br />41.2 feet. Low Volume solution collection transducer #1 was recorded at .3o and # 2 was at <br />.32. Phase II &III High Volume solution Collection transducer # 4 was recorded at 33.7 # <br />5 was at 28.0 and # 6 was at 34.5. Piezometer for the pond was at 36.0 feet. Low volume <br />transducer # 1 was at.17 and #2 was at.68. <br />Phase IV High Volume Solution Collection Transducer # 7 was recorded at 35.8, #8 was at <br />35.0, #9 was at 35.5 and # 10 was at 36.0. Low Volume transducer # 1 was at 8.2 and # 2 <br />was at 3.6 inches of hydrostatic head. The small area above (west) of the low volume <br />transducer read out location on the lined area still had some ponded water from the recent <br />storm. CC&V plans to pump the water out to the pad and try to come up with a permanent <br />solution. As reported in the last inspection, the area had received so far almost twice <br />the normal precipitation, for the year. With the rainy season for the area being from June <br />to the end of September, this area would need a permanent fix. <br />Inspected the under drain area, both the south and 4 inch pipes were discharging at about <br />the same volume noted during the last inspection. The north under drain and solid 24 inch <br />pipes were dry.
