• (Page 21 • <br />MINE ID # OR PROSPECTING ID # M-1997-084 <br />INSPECTION DATE 10/26/01 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Construction Materials 110 permits. The site <br />was not active at the time of the inspection, and the operator was not present during the inspection. <br />The site was permitted in 1997, but remained undisturbed for a couple years. Inspectors first noticed disturbance on the <br />site in March 2000, which consisted of minor earthwork on the western edge of the debris fan near the highway shoulder. <br />The operator has posted one or two signs on the site, advertising the property for sale or development. <br />There is no Sian posted here identifvina this as a site for which a MLRB permit has been issued, thouah one is required <br />pursuant to Rule 3.1.12. There are no markers which adeouately delineate the permitted area boundary, thouah they are <br />also reauired aursuant to the same rule. The lack of Sian and markers is now considered a problem, and is so noted on aaae <br />one of this report. Now that mining activities have begun, these items are reauired to be placed on the site. The operator <br />must comply with Rule 3.1.12. and provide evidence of such compliance by the corrective action date shown on the last <br />page of this report. <br />Access to the site is approved for the western edge of the permit area, directly from the highway right-of-way. Fill material <br />placed in the borrow ditch is required for such access. The annual report filed in January 2001 includes a map with a <br />notation concerning "temporary access to be constructed..." at a location matching the fill location observed on the site. <br />However, while site disturbance is still fairly minor, it appears to be more extensive than last year, and the operator is <br />reminded to fully report all new activities, disturbance and/or reclamation performed on the site in the next annual report. <br />The next annual report is due on 2/2/02, and must reflect current changes. <br />Disturbance onsite now includes more extensive borrow of material, from two slots dozed into the toe of the debris fan, <br />and its placement as a level surface along the edge of the highway shoulder, covering an area of about 0.1 acre. Very little <br />woody vegetation was removed or damaged in this activity, though any additional work there will likely begin to affect <br />wooded areas. <br />The bond for this permit is currently S1,053. The permit is now scheduled for a bond recalculation. There have been no <br />changes to the mining and reclamation plans. Since there has been only limited disturbance to-date, it appears that no <br />increase to the bond amount is presently needed. This assessment is based on a partial recalculation, which is now <br />included in the file. A full bond recalculation will not be performed by this office at this time. However, the operator is <br />reminded that all onsite activities must be reported in the annual reports, and any modifications to the approved plans must <br />be applied for and approved before implementing them. <br />There were no additional items observed.
