(Page 21 <br />MINE ID q OR PROSPECTING ID M M-77-305 <br />INSPECTION DATE 9/24/99 INSPECTOR'S INITIALS WHE <br />BSERV <br />This inspection occurred as part of DMG's routine monitoring plan for permitted operations. Dave <br />Diss accompanied the DMG inspector throughout the inspection. The CLP-21 Mine is approved <br />for 13.2 acres affected area. DMG holds 515,400 financial warranty. The permit status is <br />intermittently active. Public access to the portal is controlled by locked portal doors. <br />Signs & Markers Mine identification sign was posted at the mine entrance. Affected area <br />perimeter markers were posted. Mining activities were confined within approved areas. Mine <br />entrance road was well maintained. Public access to the portal is controlled by locked doors. <br />Facilities Affected area at the portal was visually estimated at 4 acres and included the following: <br />Shop building - 56'L x 29'W x 12'H, Butler type building with concrete foundation and floor; <br />Compressor building - 20'L x 14'W x 12'H, with concrete floor; <br />Shed-10'x10'x10'; <br />Electrical substation; <br />Water tanks -approximately 1,000 gallon and 6,000 gallon capacity; <br />Portal decline with gate - 9'x 11'; <br />Topsoil stockpile -vegetative cover well established, evidence of erosion or deterioration of <br />stockpiled topsoil not observed; <br />Waste rock stockpile - <br />Vent hole -permit documents indicate 4 vent holes, 7' diameter. <br />Financial Warranty DMG will utilize the above information and permit documents to evaluate the <br />current cost of reclamation.
