(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-~ a~~-~R'~ <br />INSPECTION DATE: ~-~8-nd INSPECTORS INITIALS: JL1^ <br />OBSERVATIONS <br />Connpleted an inspection of this operation offer the Division received a request for a partial release of <br />reclamation responsibility. <br />This is a 112 Regular permit. The size of the permlt has been in question for some time. Originally, the permit <br />acreage was 72.35 acres. According to the operator some of the acreage was released a few years ago, which <br />reduced the permit to 60 acres. The release of acreage is questionable because there is no document in the file <br />that would verify; 1) that a release request was ever submitted to the Division and 2) that the Division ever <br />approved a release request for this permit. If the operator did submit a release request please submit a copy of <br />the document to the Division (submit a copy of the Division's approval letter if it is available). <br />The annual reports are also confusing because some say the permit size is 72.35 acres and some say it is only <br />17.23 acres. The file maps are confusing too. Some show a large permit area (extending Into the Dunes golf <br />course) and some show a small pond area. The permit boundary did not appear to be marked in the field <br />during the last inspection so it was difficult to determine just what the permit area looked like and what the <br />perrnit size was. This was noted as a problem and the operator was required to delineate the approved permit <br />boundary and submit an accurate map. The operator complied with the corrective action so the problem has <br />been abated. In order to straighten out the problem, the operator decided to submit a request for an acreage <br />reduction. The acreage that was in need of additional reclamation has been Jeff in the permit area and the <br />acreage that has already been reclaimed, or never mined, is to be removed from the permit area. The operator <br />subrnttted an aerial photo delineating the revised permlt boundaries. Pence posts have been Installed around <br />the lake to delineate the permit boundary in the field. The application for partial release did not state how many <br />aere~s remain un-reclaimed (or are part of the lake). Please provide an estimate of the size of the revised permit <br />area. <br />The approved post-mining land use for the disturbed lands is a 42-acre pond and recreation areas (Nature <br />Reserves). The approved slope for the pond shores is 3:1. There are some areas of the pond that have near <br />vertical shorelines.lhe operator said these areas were already released. Again, it does not appear that a <br />release request was ever approved for this permlt. If the southern part of the pond was not released (and other <br />areas with vertical shorelines), the operator will need to grade the slopes to 3:1, or install railroad ties, to stabilize <br />the shoreline before a final release can be considered. <br />Did not observe an adequate mine ID sign of the entrance to the permit area during last Inspection. The <br />operator was required to install an adequate mine ID sign at the entronce of the permit area.
