(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-14o7_nle <br />INSPECTION DATE: 5-6-0~ INSPECTOR=S INITIALS: ~- <br />OBSERVATIONS <br />Completed an inspection of this site as part of a Mined Land Reclamation Board directive to inspect every mine <br />in the state at least once every four years. The site was last Inspected November 16, i 998. <br />This is a 75-acre quarry located about 3 miles west of Kipling street on Deer Creek Canyon road. The site is <br />currently inactive. However, it is not in temporary cessation at this time since mining activities will be Initiated <br />sometime soon, according to Mr. Peter McKenzie, company representottve. <br />The quarry operation, when active, moves in a general westerly direction. Currently, there are several benched <br />areas. The product is blasted, transported to the processing area, processed, stockpiled, then hauled offsite. <br />Washed fines are conveyed back up the mountain slope to be used as a growth medium during reclamation. <br />Bench widths are upwards of 45 feet. Highwalls are on the order of 30-100 feet. <br />An adequate mine ID sign has been placed at the entrance of the permit area, as required. <br />The fuel tank in the equipment area (5000 gallon tank) does not appear to be adequately contained. The front <br />part of the tank area does not appear to be sufficiently bermed to contain all the fuel from a large spill. The <br />containment facility must be large enough to contain the entire 5000 gallons plus have sufficient capacity to <br />contain an additional 33 percent (another 1667 gallons). This is noted as a problem on page 3 of this report. The <br />operator must submit a plan, with a specific abatement timetable, to the Division by July 25, 2004. <br />Found several drums in the equipment area that appear to contain an unknown liquid. It appears to be anti- <br />freeze or, possibly, snowmelt or rainfall. The drums are sitting on wood palettes that are not located within a <br />contained area. The operator needs to determine the kind of liquid the drums contain and move the drums to a <br />containment facility or remove them from the permit area. This is noted as a problem on page 3 of this report. <br />The corrective action due date is set for July 25, 2004. <br />The financial warranty requirement has not been reviewed for several years so the DMG re-evaluated the cost <br />of reclaiming the 20-acre phased mining area. The revised cost of reclamation is esflmated at $252,500. This is <br />an increase of $128,100. A bond increase notice will be sent to the operator pursuant to provisions of Rule <br />4.2.1(2).
