(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~Q80=~dQ <br />INSPECTION DATE: s_m_n~ INSPECTOR'S INITIALS: ~>~ <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the leach pad facility on 5-Ol -03. <br />Present during the inspection were, Phil Barnes, Mark Tidquist and Terry Mnadziac of CCBV. The inspection <br />began around the Phase IV pregnant solution collection area. Poly flex had deployed approximately 400.000 <br />square foot of 100-mil liner the past week, according, to operator's representative present. While in the area, <br />Golder and Associate's Ron Denado, who is in charge of the QA/QC personnel on site joined the inspection and <br />explained what they had done, in the past two weeks. According to Ron, most of the underground remediation <br />is almost complete and there was no differential settlement noted in the areas remediated. Along the west, East <br />and North of the pregnant solution collection area, back fill, grading, and soil liner deployment was taking <br />place. All of the lined pregnant solution collection area was covered with three feet of leak collection gravel. <br />Poly flex was deploying the top 100-mil liner on top of the leak collection gravel. While in the area, Terry <br />Mnadziac and Ron Denado mentioned that due to shortage of double textured 100 mil liner, approximately two <br />panels will only have single textured and will be placed at the bottom of the pond. Since the material is going to <br />be deployed at the bottom of the pond, the Division does not anticipate any problem. However, since the <br />APPROVED, specification called for double-sided textured material to be placed in the area, both Golder and <br />CCf:V representatives were notified to make sure and mention it in the certification report. Just for the record, <br />under Phase I, all liners were smooth textured. <br />While all representatives present, a brief discussion was held regarding the pending Certification report for the <br />Phase IV pregnant solution collection area. As was the case in the past, CC&V plans to load the area with ore <br />while the certification report is submitted and before approval is granted by the Division. The Division has <br />allowed this in the past and does not see a problem. In addition to the weekly construction reports, we receive, <br />the site was Inspected frequently, and the Division approved all the changes to the specifications in the field. <br />Plus, if a major error is found in the certification report, which is unlikely, given the above reasons, the operator <br />(CCBV) IS TAKING ALL THE RISKS. <br />Continued the inspection to the Phase I AND 11 of the pregnant solution collection areas and collected the <br />following readings from the high and low volume solution collection Transducers. <br />Phase I High volume solution collection transducer # 1 was recorded at 29.9, # 2 was at 32.9 and # 3 was at 32.4. <br />low volume solution collection transducer # 1 was of .35 and #2 was at.22 <br />(Piezometer for the pond was at 37.0 feet) <br />Phase II High volume solution collection transducer # 4 was recorded at 37.2, # 5 was at 32.4 and # 6 was t 38.9 <br />Low volume solution collection transducer #1 was at .64 and # 2 was .27 <br />(Piezometer for the pond was at 39.4 feet) ALL TRANSDUCER READINGS ARE IN FEET OF HYDROSTATIC <br />HEAD) <br />Inspected the external pond, it was holding some water, while in the area, collected the following readings from <br />the low volume solution collection transducers.
