<br />• (Page 2) <br />MINE ID ~ OR PROSPECTING ID # <br />INSPECTION DATE 3/26/97 <br />M-77305 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS WHE <br />This complete inspection was conducted by Wally Erickson from the DMG Durango office, 484 <br />Turner Drive, Building F, Suite 101, Durango, CO 81301, phone 1970) 247-5193. Glen Williams <br />and Robert Schwind, Cotter Corp Nucla office, accompanied the DMG inspector throughout the <br />inspection. The C-LP-21 Mine is located approximately 15 miles NW of Naturita, in the Long Park <br />mining district, in portions of the SW 1 /4 of the SE 1 /4 of Section 22, and portions of the N 1 /2 <br />of the S 1/2, and NE 1/4 of the NW 1/4 of Section 27, both sections in T47N, R17W, NMPM, <br />Montrose County. Permit was issued 3/31 /79 for 13.2 acres affected area. DMG holds corporate <br />surety as financial warranty, in the amount of S 15,400. Operations were dormant during the time <br />of this inspection. <br />Records -Original permit application was submitted on 9/30/77, for 9.9 acres affected area; <br />however, permit was not issued until 3/31/79. During the permit review process, DMG approved <br />two Technical Revisions, addressing relocated vent holes and roads, and one Amendment, <br />increasing affected area to 13.2 acres. Operations went into Temporary Cessation (TC) on 8/8/80, <br />due to depressed uranium/vanadium markets. On 8/31 /90, DMG approved TR-01, revising mine <br />status from TC to intermittently active. DMG records do not indicate additional modifications to <br />the permit. <br />Signs & Markers -Mine identification sign and affected area perimeter markers were observed. <br />Facilities -Public access to mine portal is controlled by locked metallic doors. Shop/office building, <br />compressor/equipment shed, and water storage tanks were well maintained. Waste rock stockpile <br />and topsoil stockpile were evident. <br />Hydrologic Balance - Rock check dams were located within the drainage channel between the <br />waste rock stockpile and topsoil stockpile. Check dams were well maintained. <br />Erosion/sedimentation, due to permitted mining activities, was not observed within the affected area <br />or to the surrounding areas. <br />I & E Contact Address <br />NAME David H.
