HHP The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMSIPOSSIBLEVlOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE The ore pile at the site was not tarped, though the approved plan requires that it be 12131/05 tarped during precipitation or when the site is unmanned. PB or PV: PB TOPIC(S): MP CORRECTIVE ACTION: The operator must tarp the ore, and provide a statement that _, _, ore will remain tarped as required by the plan. The statement must be received from the operator by the date shown at right. 2. Hydrocarbons and lead batteries are stored at the site without the required impermeable 12/31/05 secondary containment. PB or PV: PB TOPIC(S): AT CORRECTIVE ACTION: The operator must either cease storing these items at the site, _, _, or provide adequate secondary containment for all such items. A statement describing how these items will be handled (as discussed in this report) must be received from the operator by the date shown at right. The slope above the borrow area is unstable and its failure will result in offsite damage. CORRECTIVE ACTION: The operator must eliminate the potential for slope failure and/or offsite damage, by either backfilling the borrow area or amending the permit to add land to the north of the present boundary, or other reasonable alternative (proposed by the operator).
