ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-ieRn_~ea <br />INSPECTION DATE: 7l22/nd INSPECTOROS INITIALS: -EK-_ <br />OBSERVATIONS <br />The Division conducted a weekly construction progress of the Phase IVC <br />leach and area and a monitoring inspection of the site on 8/5/04,Present <br />during the inspection were Tim Comer from CC7V and Jim Dupler from the <br />Division. Inspected the Phase IVC leach pad area. Approximately 250,000 <br />square feet of material has been deployed during the last week, according <br />to Tim. The deployed liner was inspected and appeared to meet the required <br />specifications. <br />Drain cover and clay liner was also being deployed along the NORTH and WEST <br />facing slopes at the time of the inspection. Golder QA/QC were present over <br />looking the deployment. <br />Inspected the phase I, II, III AND IV leach pad pregnant POND areas and <br />recorded the following readings from the High and Low volume solution <br />collection transducers. <br />Phase I High Volume solution collection transducer # 1 was recorded at 35,5 <br />#2 was at 38.4 and # 3 was at 38.2. Low volume transducer # 1 was at .22 <br />and # 2 was at .13, Piezometer for the pond was at 38.6 feet. <br />Phase II and III High Volume solution collection transducer # 4 was <br />recorded at 34.0 # 5 was recorded at 27.8 and # 6 was recorded at 36.7. Low <br />volume transducer # 1 was at ,il and # 2 was at .28. Piezometer for the <br />pond was at 36.1 feet. <br />Phase IV High Volume solution collection transducer #7 was recorded at 22.7 <br /># 8 was at 21.9 # 9 was at 22.8 and # 10 was at 23.1. Low volume transducer <br /># 1 was at 17.8 and # 2 was at 8.2 inches of hydrostatic head. (All other <br />readings are in feet of hydrostatic head) <br />Inspected the external pond, it was holding some water (rain). While in the <br />are recorded the following readings from the Low Volume collection <br />transducers. Transducer # 1 was at 11.1 and # 2 was at # 8.2 <br />Inspected the under- drain area, only the upper pond was holding measurable <br />water. The South, North and 4-inch pipes were discharging at the time of <br />the inspection, <br />Inspected the main Cresson area, it was not active. All storm water <br />detention structures were in good repair, no major problem noted. Inspected <br />the East Cresson area, it was active, both mining and backfilling were <br />taking place, as approved in Amendment # 8. Inspected the Iron Clad and <br />Globe Hill areas, no major problem noted. Storm water diversion and <br />detention structures along the sides of the haul road in the area, appeared <br />to be in good repair, <br />
