r- • • <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-tq7~-nmHR <br />INSPECTION DATE: in-i>;-nn INSPECTOR'S INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 10-18-00. The site is a large 112-sand and <br />gravel operation quarry operation located approximately 16 miles South West of Colorado Springs. Present <br />during the inspection were Messieurs Mark Heffner and Art Simpson. The site is large 112 sand and gravel <br />operation located with in the City of Colorado Springs North of Academy blvd. The site is divided into phases <br />for mining purposes but the financial warranty is posted fnr the entire anticipated disturbance. According to <br />the annual report and what was observed on site, most of the mining last year took place in Phase III area. <br />The Northwest limit of the permit area was being back filled with wash fines. Inspected the bone yard area <br />that was back filled and reclaimed in 1985. It was being used to store some mining related equipment. <br />Inspected the East Side of Phase I and II where active mining was taking place. The material was being <br />transported to the processing area using a portable conveyer system measuring approximately 1500feet. <br />This system was not covered in the amendment and would require additional warranty for removal during <br />reclamation. Operator was notified and agreed to submit the additional warranty calculation to cover the <br />cost of removal. Inspected the Phase IV area of the permit. Along the South side of the phase IV portion of <br />the permit area, there was a water treatment plant and numerous ejection wells not associated with the <br />operation. According to the operator, the treatment facility belonged to Slag Log as part of a remediation <br />plan that was required by EPA. Operator will need to establish ownership of the facility as being Slag Log by <br />providing a Technical Revision to the permit and provide proof that facility will not be damaged by the <br />mining activity in accordance with Rule 6.4.19.
