<br />- ~> <br />(Page 2) <br />MINE ID / OR PROSPECTING ID f M-80-244 <br />INSPECTION DATE 12 1 94 INSPECTOR'S INITIALS GSC <br />This was a monitoring inspection of the Phase 1 leach pad construction. Dave Dix and John <br />Hardaway were on-site. Cat dozers were pushing drain cover fill on the southwestern and <br />northeastern elopes. Personnel and equipment at each site included one dozer, one Golder QC <br />man and 2 rock pickers. Drain cover had been previously placed in 5 foot lifts and the <br />dozers were spreading this to acquire the 3 foot drain cover specification. <br />Drain cover fill had not been placed in the area of the pregnant solution sump and adjacent <br />toe dam due to the presence of ice in the Bump. Portions of the ice were being melted with <br />15 electric stock tank heaters and the water was pumped into a Cat 773E water truck. The <br />ice, averaging about 6 inches thick, also was broken manually with sledge hammers and the <br />pieces were hand loaded into the bucket of a Cat 350 L track excavator and carried about 600 <br />to 700 feet north of the sump. The disposal of the sump water was discussed with Carl Mount <br />of the Division by telephone, The operator agreed to dispose the melt water into lined ponds <br />5 or SA on Pad 2, depending on the available room in each. At 1500 hrs. moat of the melt <br />water in the pregnant solution sump had been removed and personnel were still breaking and <br />lifting the remaining ice. <br />Juan Rodriquez was observed splicing additional cable for piezometer ~1. He expected the <br />splicing to be completed on 12-2-94 and planned to test and calibrate the piezometer. The <br />cable will be installed in a trench through the drain cover fill already in place on the <br />elope. <br />An area below panel SH184 (southwestern elope) contained ice which needed to be removed <br />before placing additional drain cover fill. It was located along the edge of drain cover <br />fill adjacent to uncovered geomembrane. <br />it was observed that the •winterization' berm was in place and the Gundel crew were <br />installing sacrificial VLDPE panels over the berm area. The panels were labeled 'to be <br />removed in Spring 1995'. Panel ~ 348 was placed at 1400 hrs. <br />The Division observed the operator flu eh the 24" diameter solid SDR 17 HDPE underdrain pipe <br />on this day. This was observed from the discharge sump below the toe dam. Dave Dix was on <br />hand at the upper sump in Arequa Gulch where a water truck poured water into the riser <br />starting at 1442 hrs. The water began discharging from the pipe below the toe dam at 1452 <br />hrs. The water appeared brown and muddy for approximately 30 seconds and began clearing up. <br />Dave Dix said that the water truck initially stirred up mud in the box surrounding the riser <br />pipe and this was flushed along with the water. By 1453 hre the water was slightly to <br />moderately cloudy in appearance.
