~ ~ deceived:
Cugnini Pit
M-1986-061
SAN 2 41Q01
~~~9o Fie
REA M~"ere/s d ~o °8Y
On October 26, 2000 personnel from the Durango Field Office of the Division of
Minerals and Geology inspected the above referenced site. The Inspection Report noted
that Lafarge had imported and stockpiled approximately 2,500 cubic yards of sand from a
site outside the permit area. The corrective action noted in the Inspection Report is to
submit the requved notice with all pertinent details. This document will serve as such
notice.
a. The stockpile of sand is located in the central portion of the site, at the south end of
Phase I.
b. Approximately 2,500 cubic yards of sand has been placed there.
c. Enclosed with this notice is an affidavit certifying the material to be free of
contaminants and inert in nature.
d. The approximate period this material was imported is from August 2000 through
September 2000.
e. The operator intends to utilize this sand as both a necessary constituent in the process
of asphalt paving material and as natural sand fill for area projects. Therefore the sand
will be removed from the site over the course of on-going and future operations. The
sand is stockpiled on disturbed ground so no additional reclamation liability has
occurred.
The south slope of the sand stockpile is reasonably close to the permit boundary. The
operator will insure that the stockpile is stable and that any possible encroachment
beyond the permit boundary be prevented. Periodic inspections of the stockpile will
be performed to maintain stability.
