(Page 2) <br />MINE ID # OR PROSPECTING ID #_ <br />INSPECTION DATE INSPECTOR'S INITIALS CBM <br />OBSERVATIONS <br />It should be noted that, although certain boxes under "General Inspection Topics" on the first page of this report are marked, <br />this in noway implies that certain violations found at previous Mined Land Reclamation Board meetings have been cured or <br />that corrective actions have been completed to the Division of Minerals and Geology's satisfaction at this time. <br />This inspection was done at the request of the Mined Land Reclamation Board to determine compliance with changed Board <br />Order due dates determined in the Board Meeting of June 14, 2005. In general, the Board delayed compliance with filing a <br />conversion application for 120 days from the original due date so long as the operator was in compliance and had completed <br />activities detailed in the interim sediment control plan. The interim sediment control plan was to be completed by no later than <br />August 1, 2005. This inspection found that the plan had been started but not completed. <br />At the Board meeting of August 31, 2005, the Board decided to change the due date for completion of the interim plan to no <br />later than September 30, 2005 since the operator presented evidence that the plan had been completed. The September 30, <br />2005 due date was to give the Division of Minerals and Geology time to conduct another inspection to determine whether the <br />plan had actually been completed to the Division's satisfaction. The operator should be aware that this would include <br />installation of drainage control measures at the lower road crossing. <br />The operator at the site was quite cooperative and wanted advice on how to complete the drainage control measures required <br />by the interim plan. Items discussed included removal of sediment from the temporary catchment basin and drainage channel <br />below the lowest mine road, installation of riprap below the road, drainage control across the road, separation of drainage from <br />mine disturbance areas and drainage from Taylor Gulch above the lowest mine road, drainage of Taylor Gulch beside the <br />upper mine access road and routing of Taylor Gulch into the roadside ditch. Pictures of these areas were taken and are <br />attached. <br />Responses to this inspection report should be directed to Carl B.
