(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1987-143 <br />INSPECTION DATE 7-25-2007 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was afollow-up to the inspection conducted on 9-7-2006 to inspect and verify the corrective actions taken to <br />resolve problems noted in that report. Russ Means with the Division was present for the entire inspection. <br />The mine identification sign and affected area houndary markers were still not observed and must be installed per the <br />requirements of Rule 3.1.12 that state: <br />1) The operator shall post a sign at the entrance to the mine site which shall be clearly visible from the access road and <br />specify the following: <br />a) The name of the operator; <br />b? indicate that a reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation <br />Board; <br />c) the permit number. <br />2) The boundaries of the affected area will be marked by monuments or other markers that are clearly visible and <br />adequate to delineate such boundaries, <br />The lack of an identification sign and boundary markers remains as a problem with the corrective actionlsl and correction <br />date specified on the last page of that previous report. The problem is not reiterated in this report, but must be <br />corrected, with photographic evidence of compliance submitted by September 30, 2007. <br />The portal collapse behind the steel door edit has been filled in and a new portal closure has been constructed. The <br />rebuilt portal is lined with stacked concrete blocks and has timbers set on the sides with locked steel gates installed. <br />The portal appears to be secure from unauthorized entry. The edit remains blocked by material from the collapse just <br />inside the new portal closure. The area around the rebuilt portal has been disturbed by excavation to fill the collapse and <br />backflll against the portal closure. This area will need to be seeded to help increase stability of these soils and steep <br />slopes. A seed mix with native grass, (orbs and shrubs is recommended. Please submit a proposed seed mix by August <br />31, 2007 for DBMS approval, so the seed can be planted on this disturbed area prior to September 30, 2007.
